A procedure for tests to be conducted on automated equipment by the operator and the Coast Guard. The procedure for tests must set forth details of the tests. Annual testing of generators must be conducted in accordance with approved testing procedures.

Rudder angle indicators must correctly reflect the actual position and be direct, reliable, and consistent at all control stations. Rudder angle indicators at the bridge did not match indicators in engineering space. Make appropriate repairs or calibration so all indicators reflect are consistent with each other and are accurate. Acceptance by the Coast Guard of automated systems to replace specific crew members or to reduce overall requirements for crew members depends upon the— (1) Capabilities of the automated system; (2) Combination of crew members, equipment, and systems necessary to ensure the safety of the vessel, personnel, and environment in each operating condition, including maneuvering. • The PSTPs were unable to be tested because the test equipment outline in the PSTPs was not on board the vessel. Complete testing of the overspeeds, low lube oil pressure (PSTP Test Number 11, page 20 of 74), and high jacket water temperature (PSTP Test number 6, page 13 of 74) alarms on all generators and mains prior to departure. Oil filtering equipment referred to in paragraph 1 of this regulations shall be of a design approved by the Administration and shall be such as will ensure that any oily mixture discharged into the sea after passing through the system has an oil content not exceeding 15 ppm. In consider the design of such equipment, the Administration shall have regard to the specification recommended by the Organization. Two quick-connect hose fittings were found on the discharge side of the OWS three-way valve leading directly overboard. The crew was using the OWS-processed water to recirculate in the OWS. Review the OWS system, to include piping, and manufacturers drawings/plans to verify the equipment is installed in accordance with MARPOL Annex I and MEPC 107(49). Submit results to OCMI for approval and operate to the satisfaction of the attending/reviewing USCG Marine Inspector. Cite: MARPOL Annex I/Reg 14.6

The training manual, which may comprise several volumes, shall contain instructions and information, in easily understood terms illustrated wherever possible, on the life-saving appliances provided in the ship and on the best methods of survival. The SOLAS training manual was not vessel specific and had lifesaving equipment not outfitted on board the vessel. Revise the SOLAS training manual and discuss updates with crew. The master shall, at least once each 12 months, ensure the performance of the tests and inspection of each portable fire extinguisher, semiportable fire extinguisher, and fixed fire-extinguishing system aboard the vessel. The last annual firefighting servicing was conducted April 5, 2024. This deficiency is issued with a due date of May 1, 2025 to allow the vessel to transit back to the US to receive annual servicing by a competent service provider. Cite: 46 CFR 131.590

A telephone operated from the pilothouse that reaches the master’s stateroom, engineer’s stateroom, engine room, and crew accommodations that either— (i) Is a sound-powered telephone. During the steering test, the sound powered telephone in engine room was inoperable. Cite: 46 CFR 130.440(b)(2)

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The stbd liferafts painters were improperly attached to the hydrostatic releases. Cite: SOLAS (2009) III/Reg 20.2
