Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. During the life raft deployment, as the cradle opened, the stainless steel wire used for the hand brake parted and was thrown on deck. This action rendered the self lowering cable inoperable.

The arrangements for the ready availability of water supply shall be in passenger ships of 1,000 gross tonnage and upwards such that at least one effective jet of water is immediately available from any hydrant in an interior location and so as to ensure the continuation of the output of water by the automatic starting of one required fire pump. PSCO observed a lack of ready availability of water supply due to the fire fighting pump being placed in manual mode. This was listed as a part of the vessel's procedures.
