The company should ensure that all personnel involved in the company's safety management system have an adequate understanding of relevant rules, regulations codes and guidelines.(ISM 6.4). During PSC Exam PSCO identified two breakers on the emergency generator switch board taped in the "off" position to prevent use. PSCO expanded the exam which revealed SMS procedure for "Lock out/Tag out" exists per (SMS 5.15). However, system is not implemented on board, nor did ship have supplies to implement Lock out/Tag out. Based on the above, an additional internal ISM Audit is required. 74SOLAS(14)IX/3.1 Ref. ISM 6.4 50(a)(c).
