Tankers shall be equipped with at least one portable instrument for measuring oxygen and one for measuring flammable vapour concentrations, together witha a suficient set of spare. Suitable means shall be provided for the calibration of such instruments. Multi-gas meters were not properly calibrated, and crew was unable to calibrate them when given the opportunity.

After any survey of the ship under regulations 7,8,9 or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the Administration. Vessel is equiped with inert gas generator. During operational testing, I.G. system shut down unintentionally three times. Oxygen analyzer for I.G. system inoperable.
