Fire hazards to be minimized. The general construction of the vessel must be such as to minimize fire hazards. MI observed machinery space hatches to be missing gaskets. Hatches also had several holes that would prevent the containment of the pre-engineered fire extinguishing system agent.

Inspection of batteries for condition and security of stowage. MI observed both port and starboard battery boxes to be missing lids.

The design of all exhaust systems must ensure minimum risk of injury to personnel. MI observed loose exhaust lagging on the port main engine.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment. MI observed #2 void bilge pump float switch to be inoperable. MI observed vessel rep correct deficiency on the spot.

The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. Inspector found multiple bulkhead penetrations that were not properly sealed to make bulkheads watertight. 46 CFR 179.320(c)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (1) Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. MI found large tears in the chine on the starboard bow and port stern with an unknown extent of damage behind them. Additionally, the chine that was repaired on the starboard stern was still leaking water. Prior to conducting any repairs, submit a repair plan to MI for review. 46 CFR 167.802(a)(1) 60c

The marine inspector may require any part or all of the propeller shafting to be drawn for examination of the shafting and stern bearing of a vessel whenever the condition of the shafting and bearings are in question. MI found that the shaft bearing on the port has excessive wastage exceeding 50%. 46 CFR 176.670(a) 60c

46 CFR 182.455(b)(4) - Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. Vessel's remote operated fuel shut off valve for port side MDE and Generator was installed so that pulling the handle did not completely shut valves.

47 CFR 80.905 - Vessels subject to part III of title III of the Communications Act that operate in the waters described in §80.901 must, at a minimum, be equipped as follows: Vessels must comply with the requirements for a navigation receiver or manual updating of position information contained in §80.1085(c). To include information in a distress alert, the VHF-DSC radio should be properly connected to a GPS receiver or have a VHF-DSC marine radio with integral GPS. Both of vessels installed radios are not displaying the GPS position.

Digital selective calling was not operable on the VHF radio.

Could not prove that losing main engine control does not increase shaft speed.

Provide 04 lifejackets approved IAW 46CFR185.604. Reduced permitted passenger count onboard to 24 passengers until deficiency is corrected.

Provide visual indication of bilge alarms for forward and aft spaces.

Provide metal clamp supports spaced not more than 24" apart in engine space for cabling.

Provide lube oil pressure indication at operating station.

Vessel is certificated to carry a total of 29 persons and has only 19 lifejackets onboard. Vessel is limited to carrying 19 passengers at all times until the amount of passengers the vessel is certificated for corresponds with the amount of lifejackets onboard.

Vessel does not have stability letter onboard. Provide valid stability letter and post it on bulkhead.

Current certificate of documentation (COD) not on board. Provide valid COD.

Safety Radio Telephony certificate expired. Provide valid certificate.

Vessel does not provide passenger count to a shoreside entity. Provide plan to make passenger count vailable shoreside.

Auxiliary portable generator cable creates tripping hazard and is in poor condition. Replace cable, provide safe walkway and remove extra extension cord.

Replace expired battery in lifering water light.

Replace plastic wire ties with metallic wire supportson bilge overhead.

Cable to fuel shut off valve requires adjustment to ensure tank is fully isolated when remote valve is operated.

Make weld repairs 02 cracks on deck and 04 cracks on handrails.

Provide marine operator log, FCC radio operator permit, FCC station licences, and FCC safety certificate.

Vsl Captain reported failure of Port engine due to lower unit locking up.
