The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. The vessel suffered a fire on March 29, 2021 in the engine room and lost primary generators and main propulsion. Status of critical systems are unknown and fixed fire protection system is at limited capacity.

Machinery Spaces of Category A shall be protected by a fixed water based local application fire extinguishing system. In the case of periodically unattended machinery spaces, the local fire extinguishing system shall have both automatic and manual release capabilities. On board water mist fire extinguishing system did not discharge automatically during generator fire on March 29, 2021. System was not operational on April 4, 2021.

VESSEL WAS UNABLE TO START MAIN ENGINES WHILE GETTING UNDERWAY. VESSEL SUBSEQUENTLY MADE IMPACT WITH THE PIER. CLASS ATTENDANCE IS REQUIRED TO ATTEST TO CAUSE AND CORRECTIVE ACTION OF MAIN ENGINE MALFUNCTION AND TO THE EXTENT OF DAMAGE FROM IMPACT.
