Independent cargo tanks shall be electrically bonded to the hull. All gasketed cargo pipe joints and hose connections shall be electrically bonded. PSCO observed missing and severed bonding on cargo tank piping, manifold piping, vapor piping, and tank cleaning piping.

Every tanked shall be provided with the means to enable the crew to gain safe access to the bow. PSCO observed catwalk grate in vicinity of No.2 cargo tank severely wasted.
