The fire integrity of the divisions shall be maintained at openings and penetrations. Self-closing door located at machinery space, "A" class boundry, aft of the main boiler, did not close. Additionally, the bulkhead penetraions had fire resistant material missing. 74 SOLAS 2004 II-2/9.1.3 10AC

Resolution: The whole section of the deck lighting fitting was renewed with new casing and connector immediately. Regular and stringent inspection will be carried out to ensure that all the electrical equipment in any hazardous zone is properly maintained.

Resolution: The section of the inlet water line was taken out to engine workshop and the section of pin holed area was replaced with a new section of pipe. After fitting back the line was tested with working pressure and no further leak was found.

Resolution: The suspected leaking valve was renewed with new one. The line was opened up at the flanges and entire system was flushed at high pressure. After buffing and pressurizing the line, no leak or remarkable wastage of material found.

Resolution: After line blowing through for a prolong period, the equipment was showing correct Oxygen level and tested high Oxygen level alarm in good order. Additionally, two new spare Oxygen sensors arranged to be supplied at Panama on 7th November for future use
