Every oil fuel pipe, which, if damaged, would allow oil to escape from a storage, settling or daily service tank situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. If such additional valve is fitted in the machinery space it shall be operated from a position outside the space. PSCO observed a threaded bolt positioned in between the quick closing valve for the HFO settling tank preventing the valve from being closed by the crew outside of the space. The crew removed the bolt and were still unable to close the valve because it was seized in place. The crew used tools to forcefully close the valve.

PSCO identified that all auxiliary engines are operated on a single seawater cooling pump due to an inoperable secondary G/E saltwater cooling pump. Loss of the single seawater cooling pump would result in a loss the generating set which serves as the main source of electrical power to the vessel.

Each completed operation shall be signed by the officer or officers in charge of the operations concerned and each completed page or group of electronic entries shall be signed by the master of ship. PSCO discovered the following ORB part I deficiencies (a-c):
(a) The crew is using the Bilge Separator Oil Tank (BSOT) to retain accumulated sludge in the engine room. The BSOT is listed under section 3.3 of the supplement to the International Oil Pollution Prevention Certificate which is reserved for collection of oily bilge water. The BSOT is not listed as a sludge retention tank under section 3.1 of the supplement to the International Oil Pollution Prevention Certificate. Multiple entries for sludge retention in the BSOT were found in the ORB part I from 2022 to present day. (b) The last entry in the Oil Record Book for retention in the Bilge Separator Oil Tank (BSOT) is 4.6 m3 on December 2, 2023. The PSCO witnessed the crew sound the BSOT on December 6, 2023 and the retention is 2.25 m3. The Chief Engineer stated that approximately 2.35 m3 of oily water was transferred by the crew from the BSOT to the main engine settling tank and the water was boiled off. The transfer of oily water from the BSOT to the main engine settling tank was not recorded in the ORB part I. (c) The crew is recording tank to tank transfers in the ORB part I using code D, 15.1 which is reserved for the use of the oily water separator 15 ppm operation. Multiple tank to tank transfers were found with the wrong code between 2022 to present day.

PSCO observed machinery and piping leaking excessive water into the engine room bilge. The Cargo Air pump was found leaking while in operation at an approximate rate of ½ gallon per minute. The G/S fire pump was found leaking at the gland seal at a rate of ½ gallon per minute while in operation. The emergency fire pump suction piping was found leaking at the seawater skin valve at a rate of 1 gallon per minute. All engine room bilge wells were found at capacity with oily water mixtures. All sludge retention tanks were found to be at or close to capacity.
