PSCO OBSERVED ACTIVE LEAK WITH SEVERAL (MIN 1-2 DROPS PER SECOND) DROPS OF FUEL OIL MIXTURE FROM FUEL PUMP BOX PIPING SYSTEM ON MAIN ENGINE. 74 SOLAS(04) II-1/26.1

Resolved 3 May 2021
Resolution: The soaked lagging was removed, internal areas thoroughly cleaned, and hardware tightened as applicable. Fuel oil was circulated through the system, and no leaks were noted. New lagging was installed within the box and along with small sections of pipe.
