All ships of 3,000 gross tonnage and upwards shall have a 3 GHz radar or, where considered appropriate by the Administration, a second 9 GHz radar, or other means, to determine and display the range and bearing of other surface craft, obstructions, buoys, shorelines, and navigational marks to assist in navigation and in collision avoidane. The vessel's S-band radar is inoperable.

Before the ship leaves port and at all imes during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed a broken zipper on one of the immersion suits on the bridge. Crew replaced with a spare.

After any survey of the ship under regulations 7, 8, 9, or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey without the sanction of the administration. PSCO observed a non approved alteration of scavenging air box drain. Crew removed non approved alteration and equipment matches approved drawing.

Action required: 10 - Deficiency Rectified
Resolved 12 April 2022
Resolution: Crew removed alteration and restored to original condition.
