Electrical installations shall be such to ensure the safety of passengers, crew and ship from electrical hazards. PSCO found duct sealant used as packing material on electrical connection into fuel pump; duct sealant had dried and separated from the stuffing tube. The insulation on the cable had chaffed, exposing metal conductors within the wires.

Resolution: Ships force electrician unwired connection, renewed stuffing tube and packing, as well as connector for incoming control wires, and reinstalled to the satisfaction of the PSCO.

Special consideration shall be given to the malfunctioning of: the fuel oil supply systems for boilers or engines. PSCO observed multiple leaks in the fuel oil purifier room, fuel pooling under the F/O manifold, leaks under the controller units, and tin cans used as drip catchers.

The fire resistance of doors shall be equivalent to that of the division in which they are fitted. PSCO observed two (02) class "A" fire doors with broken latches that were unable to latch shut.
