Electrical installations shall be such to ensure the safety of passengers, crew and ship from electrical hazards. PSCO found duct sealant used as packing material on electrical connection into fuel pump; duct sealant had dried and separated from the stuffing tube. The insulation on the cable had chaffed, exposing metal conductors within the wires.

Special consideration shall be given to the malfunctioning of: the fuel oil supply systems for boilers or engines. PSCO observed multiple leaks in the fuel oil purifier room, fuel pooling under the F/O manifold, leaks under the controller units, and tin cans used as drip catchers.
