At least 30 days prior to the expiration of the Certification of Inspection, a vessel's owner, charterer, managing operator, agent, master or individual in charge shall notify the Coast Guard if the vessel will be required to be reinspected for certification or will be operated in such a manner as to not require a Certificate of Inspection. The responsible party failed to notify the Coast Guard that the vessel will require reinspection.

Resolution: Completed Inspection for Certification.
Except as provided in paragraphs (b) through (g) of this section, each tank vessel must undergo drydock, internal structural, and cargo tank internal examinations as follows. The vessel failed to undergo an Internal Structural Examination within the required freshwater service interval due on June 30, 2024.

Vsl was reported with damage received from Hurricane Ida.

Resolution: A third party damage survey of the barge was conducted by [name withheld] Marine Surveying Inc. on November 5, 2021. Attended vessel and verified satisfactory repairs of damage from Hurricane Ida.
