Where the atmosphere in double hull spaces cannot be reliably measured using flexible gas sampling hoses, such spaces shall be fitted with permanent gas sampling lines. the configuration of gas sampling lines shall be adapted to the design of such spaces. the M/T Centennial Sapporo utilizes the RIKEN KEIKI RM-950 Gas Sampling system to achieve this requirement. During the gas sampling portion of the exam, the [name withheld] stated to PSCOs that "they are only required to test gas sampling alarm set points with span gas on a monthly basis, they do not calibrate sensors." The Vessel's Safety Management System refers gas sampling system maintenance and calibration to the manufacturer's manual, which states "it is necessary to perform gas sampling system gas calibration every 6 months." The Company should identify equipment and technical systems, the sudden operational failure of which may result in hazardous situations. The vessel's Safety Management System refers gas sampling system maintenance and calibration requirements to the manufacturer's manual, which states "it is necessary to perform gas calibration every 6 months, using required calibration equipment." PSCOs identified that the last recorded span gas calibration was 27AUG2021. Vessel is currently outside required calibration window. In addition, after review of the vessel's planned maintenance system records - specificially task code OFF_000VY, gas sampling system span gas calibration took place onboard up until February 2021, after this time/date, sensor calibration ceased with no recorded change in planned maintenance system procedures. The vessel operator is required to perform an internal audit and analysis of the vessel's safety Management System focusing on the fixed gas sampling system maintenance and calibration cycle. The condition of the ship and its equipment shall be maintained to conform with the provisons of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. 0599 - In tankers, electrical equipment, cables and wiring shall not be installed in hazardous locations unless it conforms with standards not inferior to those acceptable to the organization. PSCO found 07 explosion-proof/flame proof light in hazardous areas(06 in pumproom, 01 in SOPEP locker) with cracks in housing. PSCOs found 14 additional lights where threaded stuffing tube fittings could be loosened by hand. This provides objective evidence that the lighting does not meet IGC 60092--502 requirements. 74 SOLAS (2014) II-1/45.11 50ac - removed COC from vessel.
