The construction of doors and door frames in A class division, with the means of securing them when closed, shall provide resistance to fire as well as the passage of smoke. PSCO observed A-60 self-closing doors compromised with holes and compromised latches on A-deck port and starboard doors to embarkation. Stairwell door on A-deck has compromised latch. B-deck aft port door to ladder well has compromised latch. C-deck aft starboard door to ladder well is hard to open. Steering gear room door does not self close. Door required to be self-closing shall not be fitted with hold-back hooks. Paris MOU PSC Exam on 15 MAY 2024 identified “several self closing fire doors… equipped with hold-back hooks.” The crew submitted documentation to the PSC that “all areas to be checked by safety officer,” and the deficiency was cleared. During current USCG PSC Exam, found 02 additional self-closing doors (B-deck, House entrance Port & Starboard) to be fitted with hold-backs. 74 SOLAS (04) II-2/9.4.2.2 30ac

The machinery, boilers and other pressure vessels… shall be so installed and protected to reduce to a minimum any danger to persons on board. Auxiliary Boilers No. 1 relief valve found arranged with lifting handle improperly aligned. A duplicate set of fire control plans or a booklet containing such plans shall be permanently stored in a prominently marked weather tight enclosure outside the deck house for the assistance of shore-side fire-fighting personnel. Fire control plan located at Port and Starboard gangway do not match the fire control plans inside the ship; specifically the slop chest door and the upper deck Engine Room changing room/Engine Room entrance. 74 SOLAS (04) II-2/15.2.4.2 60ac

The Administration may permit the use of oil-level gauges with flat glasses and self-closing valves between the gauges and fuel tanks (arrangements for lube oil shall at least comply with 2.2.3.5). For any vessel that is equipped with ballast water tanks and bound for ports in the U.S. submit the ballast water report no later than 6 hours after arrival. Vessel did not submit ballast water report until prompted by the USCG, Aprox 49 hours after arrival. 33 CFR 151.1060(b)(3) 10c

Each survival craft shall be stowed in a state of continous readiness so that two crew memebers can carry out preparations for embarkation and launching in less than 5 minutes. Portside lifeboat unable to be lowered in 5 minutes; lifeboat lowered in 20 minutes. Crew last lowered lifeboat on 08MAR2022, crew excessively greased the falls and drum 22MAR2022.

Every crewmember with assigned emergency duties shall be famliar with these duties before the voyage begins. PSCO observed fire team unfamiliar with operation of equipment including energizing flashlights, properly securing the SCBA, and donning firefighting outfits.

DOORS REQUIRED TO BE SELF-CLOSING SHALL NOT BE FITTED WITH HOLD-BACK HOOKS. HOWEVER, HOLD-BACK ARRANGEMENTS FITTED WITH REMOTE RELEASE DEVICES OF THE FAIL-SAFE TYPE MAY BE UTILIZED. PSCO OBSERVED SELF CLOSING DOORS FROM ENGINE ROOM LEADING TO STEERING GEAR ROOM PROPPED OPEN. PROVIDE CORRECTIVE ACTION PLAN FROM COMPANY WITH CONCURRENCE FROM ADMINISTRATION.

Drills shall, as far as practicable, be conducted as if there were an actual emergency. Crew demonstrated a lack of competency during the fire drill; 02 crew were unable to properly don their firefighter outfit and missing key equipment: firemans axe. 01 Crewmember had fire pants situated as to expose him to potential flames.
