Resolution: A Class report was provided attesting to cause being a failure to the No.8 cylinder from a leaking seal of the exhaust valve hydraulic piston. The vessel replaced the leaking seal, and the main engine was tested and found operational.

PSCO was notified that vessel had a loss of propulsion due to cylinder failure. Vessel conducted repairs and obtained class survey attesting to repairs. When transiting out of the port, the vessel had the same failure on cylinder No.1 about 5 nautical miles from shore. 74 SOLAS (20) II-1/31.1
