The machinary, boilers and other pressure vessels associated piping systems and fitting shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a mimimum any danger to persons on board...PSCO observed hydraulic oil leakage from cylinder ram on number 1 and 2 steering gear units, and from the oil seal of number one steering pump. The PSCO Discovered 09 immersion suits where the zipper detached from the suit that would allow for the ingress of water and prevents the suit from achieving a water tight seal.

High pressure fuel delivery lines between the high pressure fuel pump and fuel injectors shall be protected with a double jacketed piping. The high pressure fuel line on the No. 2 cylinder was damaged with a 1" hole in the outer jacket and was covered up with tape.
