Thermal fluid heaters are examined to determine that the complete unit is in a safe and satisfactory condition. Vessel rep reported leak in #2 port cargo tank thermal oil coil in bottom of tank. Vessel restricted from carrying cargo in #2 port cargo tank, and restricted from carrying cargo that requires a fully operational thermal fluid heater. ABS to send report attesting to satisfactory condition of vessel upon completion of repairs.

Any tank vessel which is equipped with an internal combustion engine driven cargo pump on the weather deck shall be provided with a minimum of one remote manual shutdown station, conspicuously marked, and located at the midpoint of such vessel, or 100 feet from the engine, whichever is the more practical. The remote quick acting manual shutdown shall be installed on the engine so as to provide a quick and effective means of stopping the engine (such as by cutting off the intake air). MI observed that the inboard cargo pump was not capable of being tested due to ongoing maintenance. Testing must be completed prior to using this motor for cargo operations. 46 CFR 32.50-35(a)

Operational tests and checks of all safety and limit controls, combustion controls, programming controls, and safety relief valves must be conducted by the owner, chief engineer, or person in charge at the inspection for certification, periodic inspection, and when directed by the Officer in Charge, Marine Inspection, to determine that the control components and safety devices are functioning properly and are in satisfactory operating condition. MI observed that neither TFH was capable of being tested due to ongoing maintenance. Vessel is restricted from carrying elevated temperature cargoes. 46 CFR 32.35-1 46 CFR 61.30-20

A fixed pressure-sensing device must be installed as close as practicable to the vessel vapor connection on a tank barge with a vapor collection system. The pressure-sensing device must measure the pressure vacuum in the main vapor collection line and have a pressure indicator located where the cargo transfer is controlled. MI observed that the port side vapor line did not have a pressure gauge. 46 CFR 39.2015

Piping systems used where the fluid temperature exceeds 150 °F must be suitably insulated as necessary to preclude injury to personnel. MI observed all cargo piping insulation had been removed, and new insulation was not installed. 46 CFR 32.35-1 46 CFR 56.50-1(k)

Vapor collection piping must be electrically bonded to the hull and must be electrically continuous. MI observed all PV valve bonding straps were missing. 46 CFR 39.2001(c)

The boundary bulkheads and decks separating the accommodations and control stations from cargo, and machinery spaces and from galleys, main pantries and storerooms other than small service lockers shall be of “A” Class Construction. MI observed electrical thru-hull penetrations between the BOSN store room to the TFH room were not appropriately sealed. 46 CFR 32.57-10(c)

Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port. In addition, the specified number of ring life buoys on board must be IAW NVIC 2-81ch2. MI observed that only 3 life buoys were present, when 5 were required, and all needed updated markings. 46 CFR 199.70(a)(2) NVIC 2-81 CH. 2

46 CFR 50.05-10 When alteration or repair of piping systems becomes necessary, the work shall be done under the cognizance of the Officer in Charge Marine Inspection. Vessel has a leak in cargo piping on deck. CODE 705: Vessel is authorized to enact temporary repairs to the satisfaction of class. Vessel is authorized to discharge cargo on board once temporary repairs are approved and shall enact permanent repairs to the satisfaction of attending USCG Marine Inspector prior to carriage of cargo.

The high-level alarm required by paragraph (a) of this section must— activate an alarm once the cargo level reaches 95 percent of the tank capacity or higher, but before the tank overfill alarm. Vessel representatives were unable to test high-level and overfill alarms for #2 stbd cargo tank, #4 stbd cargo tank, and #4 port cargo tank. Vessel must rectify to the satisfaction of the Coast Guard prior to loading cargo. Code705C- Next Scheduled Drydocking

46 CFR 50.05-5 (a) Whenever doubt exists as to the safety of an existing piping system, the marine inspector may require that it be gaged or checked to determine the extent of deterioration. MI observed localized pitting in cargo pipe 90-degree elbow near STBD manifold. Vessel shall gage piping to determine extent of deterioration and submit report to local USCG OCMI.

All tank vessels, the construction or conversion after July 1, 1951, shall conform to the requirements in this subpart in construction of hulls. Barge representative reported a pinhole on the raised cargo trunk main deck at the number 4 port cargo tank. Permanent repair shall be completed prior to the onload of cargo in the number 4 port cargo tank to satisfaction of attending CG Marine Inspector.

46 CFR 42.15-45(d) Except as provided in paragraph (c) of this section ventilator openings shall be provided with efficient weathertight closing appliances. The relaxation of the requirement for closures on ventilation with coamings exceeding the specified heights for positions 1 and 2 is generally confined to stand alone or free standing ventilation situated on the weather deck and not those situated within the bulkheads of a deck house or superstructure. Replace two wasted gaskets on ventilators located above the port side machinery space.

46 CFR 31.35-1 Ground indicators must be at the vessel's ship's service generator distribution switchboard for the normal power, normal lighting, and emergency lighting systems; and be readily accessible. The system must have suitably sensitive ground detection system which indicates current in the ground connection, is able to withstand the max available fault current without damage, and provide continuous indication of circuit status to ground. Identified "C" phase bulb extinguished on the ground fault detection system located at the vessel's ship's service generator distribution switchboard. Troubleshoot extinguished bulb and provide outcome.

When minor alterations or minor repairs of tank vessels become necessary such work shall be under the direction of the Officer in Charge, Marine Inspection, and shall be in accordance with the regulations in effect at the time the vessel was contracted for or built, or in accordance with the regulations in effect for new construction insofar as possible. Vessel reported a seized anchor windlass, which made it unable to pull up anchor. Prior to movement from MM 14, provide class report attesting to the satisfactory repairs and operational test of anchor windlass to the USCG Marine Inspector. 46 CFR 30.01-10

Vessel lost single anchor while anchored at AMA Anchorage MM115. Your vessel is restricted to the [name withheld] Wharf until 12AM on 5/17/2016 at which time you will be allowed to transit to Marathon Oil at MM14 vessel must return to the [name withheld] Wharf until the following conditions have been met: See Attached CG_835 dated 16MAY2016.

VCS line was not properly painted and marked according to 46 CFR 39.20-1(d).

Permanently attached 0.5 inch diameter stud projecting outward from the top of the flange face was missing on the forward stbd VCS line. Provide required stud per 46 CFR 39.20-1(e)

An extension cord was permanently installed to provide power for an overhead light. Remove extension cord and replace with approved boat cable per 46 CFR 111.60-11

Exhaust leak was detected while testing Thermal Fluid Heater. Require repairs to leaking TFH prior to use.

Lights were expired on all lifejackets. Replace with serviceable lights. per 46 CFR 199.60(b)(4)

Provide a permanently attached 1/2 inch stud at least 1 inch long at the 12 o'clock position of the vapor hose reducers attached to the port and starboard aft manifolds prior to using the 8" reducers for vapor connection. Barge had removed the studs previously due to improper installation preventing alignment with facility hoses.

Provide repair proposal and conduct repair to port aft deck area (deck, railing, mooring equipment) as well as 1" x 3" puncture to the aft port voice at the turn of the deck and the side shell.

Damaged area on Port Stern at deckline about 20 foot by 1 foot with a 3 inch hole. Provide repair plans to satisfaction of MI prior to departing Corpus Christi, TX.

Make permanent repairs to the Side Shell IWO the Damaged area at Frame 23 aft end of the Port side # 2 Ballast tank.. Area is to be Inspected during the next CG attendance.

Make permanent repairs to the Guard Railing IWO the Damaged area at the Port side # 2 Ballast tank. .
