The main inlets and outlets of all ventilation systems shall be capable of being closed from outside the space being ventilated. / PSCO witnessed two crewmembers unsuccessfully attempt to close three cargo hatch ventilation closures amidships on the port side on cargo holds 3 and 4 due to improper seating, inadequate lubrication, and general lack of preventative maintenance despite having been logged as checked and in good working order three days prior. In one instance, the frame of the ventilation and adjacent hinge were deformed and no longer seating correctly. 74 SOLAS (2001) II-2/16.9

In one box, the outlet was inappropriately mounted with electrical tape. In another, the bottom hinge was completely broken off and the upper hinge was seized with rust. 74 SOLAS (2001) II-1/45.5.5
