Each towing vessel must be fitted with a general alarm that...has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light. Vessel did not have a working red flashing light installed in the engine room.

Piping and machinery components that exceed 220 °C (428 °F), including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Port main diesel engines was found with a missing section of exhaust lagging where engine room meets main deck.

Employers must maintain records IAW 49 CFR 40.333; records shall ID the total number of individuals chemically tested annually for dangerous drugs. Current list of crew members enrolled in drug and alcohol program was not available for inspection. Provide current list.

An approved 160-B semi-portable fire-extinguishing system to protect the engine room must be equipped. No 160-B fire extinguisher was located in the engine room. O/O to permanently install a 160-B fire extinguisher in or near engine room.

Each towing vessel must have the following alarms provided: aux gen engine low lube oil pressure, aux gen engine high cool water temp. Crew was unable to test newly installed gen alarms. O/O to ensure new generator alarms are operable and tested to the satisfaction of a CGMI.

Nonmetallic flexible hose complying with SAE J1942 is acceptable fuel piping. New gens did not have approved hoses installed. O/O to install approved fuel hoses.

Machinery and fuel tank spaces must be separated from accommodation spaces by bulkheads. Overhead in engine room is wasted and open into accom space. Crop and renew wasted overhead in engine room.

Verification of compliance with the construction and arrangement design standards of this part must be performed on a vessel on which a new installation that is not a "replacement in kind" before the new installation is performed. New generators, not "replacement in kind," were installed on the vessel without verification of compliance with design standards. O/O to provide plan approval for gen swap and CGMI to inspect to satisfaction prior to movement.

Each person in charge of the navigation or maneuvering of a towing vessel of at least 8 meters (26 feet) in length must satisfy the requirements of §15.805(a)(5) of this part or hold a license or MMC authorizing service as either— (1) Mate (pilot) of towing vessels. The mate onboard has an expired license and expired medical certificate. Provide objective evidence of a properly credentialed mate, to meet the manning levels of the COI or provide objective evidence that the vessel's operations will be no more than 12 hours in every 24 hour period and within 14 days request the minimum safe manning level on the COI be reduced by one mate.

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. The first aid kit located on the bridge had items that expired in 2002 and 2005 and the first aid kit in galley/mess deck had numerous items missing. Provide objective evidence that an appropriate first aid kit is onboard.

Any piping system that penetrates the hull below the waterline must be fitted with an accessible valve, located as close to the hull penetration as is practicable, for preventing the accidental admission of water into the vessel either through such pipes or in the event of a fracture of such pipe. The valve must be clearly marked by labeling or color coding that enables the crew to identify its function. Grey water overboard piping located on port side forward outboard of sewage tank is deteriorated with synthoglass wrapping from hull extending inward 2 feet. Also has flexible hose attached to a "T" fitting with an expandable plug. Replace grey water. Renew piping section with acceptable labeled overboard valve and remove hose section and plug piping.

Vessel has a 1 inch pipe with valve going nowhere located on port side E/R bulkhead near overhead aft of grey water overboard piping. Remove valve and pipe section and plug sideshell penetration.

There must be an installed or portable bilge pump for emergency dewatering. Any portable pump must have sufficient hose length and pumping capability. All installed bilge piping must have a check/foot valve in each bilge suction that prevents unintended backflooding through bilge piping. Engine room bilge suction is clogged and missing check/foot valve. Prove proper operation of installed E/R bilge piping.

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. Port and Starboard fuel valves located in forward bulkhead of engine room in front of stbd generator and MSD are leaking fuel from valve stems. Repair/renew valves.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Port engine starting battery positive terminal is badly corroded and 1 wire is showing through insulation. Clean and re-install battery connections.

Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. Overhead wiring in engine room is supported by plastic zip-ties only. Remove all plastic zip-ties and renew with metal ties or strapping.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. #20 breaker in 120 volt breaker panel is tripped and inoperable. Remove/renew breaker.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Potable water pipe penetrating forward bulkhead starboard in front of stbd generator has rubber pipe patch installed. Renew piping section from joint to union.

The owner or managing operator of a vessel equipped with an EPIRB must maintain valid proof of registration. The registration form was available but did not indicated the vessel's call sign nor MMSI and the registration letter and decal was not available for inspection. In addition, the decal on the existing EPIRB was expired. Provide objective evidence of EPIRB registration.

MI notice vessel's stern light was inoperable.

MI found that the port side life ring was dry rotted.

MI found multiple lifejackets were missing lifejacket lights.

MI found that the hydrostatic release on EPIRB was past expiration date.

Vessel could not provide valid proof of registration for the EPIRB.

Vessel did not have RPM and cooling water gauging for the mains and did not have RPM gauging for auxiliary generator engine locally.

MI discovered all berthing areas had no form of emergency lighting.

Vessel was hauled out at Blakley Boatworks to make repairs to the starboard shaft seal. The seal failed and was allowing water intrusion into the stuffing box, in the engine room. Repairs shall be made and inspected to the satisfaction of the attending Coast Guard Marine Inspector, prior to going back into operation.

Fire pump was found inop. Prove proper operation of fire pump.

Provide waterproof container for distress flares.

Provide approved brackets for 5 portable fire extingushers .

Provide missing smoke detectors in berthing areas.

Replace cracked hydraulic lines for steering gear.

Clear debris from engineroom bilge.

Prove proper operation of Port Gen Low Oil Level alarm.

Provide covers (2) for lights in fwd hold.

Prove proper operation of stbd sea chest valve for fire pump.

Clear items stored in front of B-160 fire extingusher.

Provide fire detection servicing record.

Engine roomm fire alarm strobe was inoperable. Prove proper operation of eng room fire alarm strobe.

Provide stern light - white.

Provide VHF radios with MMSI numbers.

Provide updated charts for area of operation.

Secure exhaust lagging for both gens.

The name of the vessel needs to be marked on the sides port and starboard side of the hull.

Life rings need be orange and the vessel name must be marked on the life ring.

Fire hose needs to be place back at the fire station.

The fire extinguser in the engine room needs to be replaced.

Vessel needs a mechanical fuel shutoff system.
