46 CFR 144.205(c);701ac Propulsion machinery including main engines, reduction gears, shafting, bearings and electrical equipment and systems, must: (1) be maintained to ensure proper operation, (2) Be suitable for route and service; and (3) have suitable propulsion controls to provide the operator full control at each operating station. Vessel experienced a loss of control the the STBD Z-drive. Make repairs to the satisfaction of the TPO prior to operations.

MI observed fuel leaks on fuel transfer pump, fuel transfer pump discharge valve, lower fuel transfer manifold, and port main engine fuel filter housing. Enact permanent repairs to the satisfaction of ABS.

MI observed multiple exhaust leaks on both main engines. Enact permanent repairs to the satisfaction of ABS.

46 CFR 140.500(b) The owner, managing operator, or master must ensure that all persons on board towing vessel comply with the health and safety plan. Vessels master and crew failed follow proper maintenance and repair request procedures for oil and fuel leaks in the engine room, IAW EMI Policy & Procedures Manual. Due to the number and nature of deficiencies found, objective evidence exists that the vessels towing safety management system is not being implemented properly. An external TSMS audit shall be conducted to the satisfaction of USCG prior to departure.

Navigational safety equipment such as radar, gyrocompass, echo depth-sounding or other sounding device, automatic dependent surveillance equipment, or navigational lighting that fails during a voyage must be repaired at the earliest practicable time. Vessel reported gyrocompass equipment incorrectly transmitting. Vessel to provide TPO report attesting to proper repair and operation.
