Vessels master and crew failed follow proper maintenance and repair request procedures for oil and fuel leaks in the engine room, IAW EMI Policy & Procedures Manual. Due to the number and nature of deficiencies found, objective evidence exists that the vessels towing safety management system is not being implemented properly. An external TSMS audit shall be conducted to the satisfaction of USCG prior to departure.
