No major repairs or alterations affecting the safety of the vessel with regard to the hull shall be made without the approval of the [name withheld] observed temporary repair located on the STBD FWD side shell. Vessel to submit repair proposal prior to commencing repairs.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its COI giving consideration to provisions for a seaworthy hull. Found cracks in welds located on superstructure and on toward hatch. Found wasted deck in the main deck below the house. Repair cracks on house and forward hatch, provide repair proposal and UT shots for wastage in the engine room below decks.

In addition to meeting 180.130 each life float and buoyant apparatus must be stowed as required under this section. Marine Inspector found the life float to have wasted grab lines, faded international orange paint, and an improper weak link. Vessel must replace items to meet stowage standard.

Lifeline must be of dark color or be of a type certified to be resisted to deterioration from ultra violet light. Marine Inspector found lifeline on ring buoy to be deteriorated. Replace lifeline to the satisfaction of the MI.

Vessel operated solely within 20 nm of land must be equipped with a VHF-DSC radio telephone installation meeting the requirements of 80.110(c)(2). Marine Inspector identified VHF-DSC capabilities were inoperable due to MMSI not being entered into the radio. Ensure MMSI numbers are entered into the radio and DSC capabilities functional, satisfactory to the Marine Inspector.

The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters and shall be of such size and design as to prevent reduction in the net cross-sectional area of the vent pipe. Marine Inspector found clogged flame screen. Vessel must clean or replace flame screen.

1'x1' hole rusting the deck of the accommodation space above causing more wastage.Nothing in this part shall be construed as limiting the marine inspector from making such inspections as are reasonable and practicable to be assured of the seaworthiness of the vessel. Self reported: Vessel had a holed deck area approx 4'x4' iwo aft leg, submit repair plan for approval before conducting repairs, to the satisfaction of the attending Marine Inspector.

Nothing in this part shall be construed as limiting the marine inspector from making such inspections as are reasonable and practicable to be assured of the seaworthiness of the vessel. Vessel had multiple holes in the hull and deck: 1) Port side 1"x1" through hull in engine room. 2) STBD side engine room, top welds, contains multiple small holes spanning 2 to 3 inches. 3) STBD side overhead deck in the back engine room has a 1'x1' hole rusting the deck of the accommodation space above causing more wastage. 4) The exhaust pipe, that goes through the deck in the back aft of the engine room, is cracked all around. 5) Port and STBD side of the engine mount is wasted away and cracked. 6) Multiple wasted/cracked welds forward of the engine room. Submit repair plan for approval, before conducting repairs, and conduct a survey of the main structural members to the satisfaction of the attending Marine Inspector.

The Marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. Engine room had excess oil over machinery and the bilge area. Clean to the satisfaction of the attending Marine Inspector.

All electrical installations shall be in accordance with sub J. Lights in the engine room did not have covers or protection required by 46 CFR 111.75-20(b). Fix to the satisfaction of the attending Marine Inspector.

The fixed power bilge pump shall be self-priming and may be driven off the main engine or other source of power. It shall be permanently connected to the bilge main and may also be connected to the fire main. If of sufficient capacity the power bilge pump may also serve as a fire pump. Bilge pump was not mounted and was contaminated with waste oil, clean and mount bilge pump to the satisfaction of the attending Marine Inspector.

Vessels shall be inspected for compliance with the standards required by this subchapter. For items not covered by standards in this subchapter, inspection shall be in accordance with standards acceptable to the Officer in Charge, Marine Inspection, as good marine practice. The rudder packing flange was not double bolted, fix to the satisfaction of the attending Marine Inspector.

The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch at the pump outlet. The pump outlet shall be fitted with a pressure gage. Fire pump was fitted with a pressure gage that is not best suitable for reading the psi. Replace to the satisfaction of the attending Marine Inspector.

Vessels shall be inspected for compliance with the standards required by this subchapter. For items not covered by standards in this subchapter, inspection shall be in accordance with standards acceptable to the Officer in Charge, Marine Inspection, as good marine practice. Hydraulic pipes where leaking in multiple places of the engine room; 1) the STBD front side of the engine room, 2) at the PTO off the Main Engine. Fix to the satisfaction of the attending Marine Inspector.

The inspection and maintenance of the equipment listed in paragraph (a) of this section shall be logged or otherwise documented for review by the Coast Guard upon request. Conduct and record lifesaving maintenance; last maintenance log entry was dated July 2021.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Conduct and record drills; last recorded drill was July 2021.

In general, compliance with the standards established by a recognized classification society will be considered satisfactory evidence of the structural adequacy of a vessel. Marine Inspectors (MI) noted several structural members that were wasted and/or detached. In addition, the deck plating that was recently replaced was not repaired with CG oversight. The MI noted porosity, undercut, and welds without full penetration on several areas of the repair and sections of the plating not welded to the framing. Make repairs to noted structural members and grind/reweld recent repair to the satisfaction of attending MI.

All machinery essential to the routine operation of the vessel shall be checked. Cranes should be inspected and tested in accordance with the requirements of API RP 2D, First Edition (Oct. 1972) with supplement 1; except that the rated load test must be performed in accordance with 46 CFR 107.260 (every 60 months). Documentation showed that a third-party technician serviced the crane two weeks ago. The crane is leaking hydraulic fluid from the ram and there is a hydraulic hose that is dry-rotted and unservicable. The load test was only conducted at minimum angle. Make repairs and test at load test al all required parameters.

A vessel at anchor shall exhibit an all-round whit light. The vessel's anchor light was inoperable. Repair to satisfaction of attending MI.

A vessel must carry either a first aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Items in the vessel's first aid kit is expired. Replace expired items or procure new first aid kit.

Vessel's FCC radiotelephony certificate was expired. Provide valid radiotelephony certificate to the satisfaction of the attending marine inspector.

Vessels must comply with the requirements for a navigation receiver or manual updating of position information contained in § 80.1085(c). Vessel's DSC capable radio could not display position information. Rectify deficiency via one of the following ways: *Install new radios with built-in GPS *Connect GPS to existing radio given radio will accept

All wiring shall be suitable for marine service. MI noted a bare wire on the MDE alternator not connected, another on the starter, and an electrical receptacle in the engine compartment with no cover.

Vessels in lakes, bays, and sounds service shall carry sufficient life floats or buoyant apparatus for not less than 30% of all persons on board. The vessel's owner removed the life float. Install life float or buoyant apparatus to satisfaction of MI.

MI observed numerous corrosion issues, including holes and heavy corrosion on deck longs, framing, and side longs and plating. Vsl must be surveyed prior to MI attending to conducted a ISE for credit. Vsl is restricted from operations, but may transit to Port Sulfur to effect repairs. 46 CFR 177 17C

MI observed missing report for the 5 year leg inspection. Vsl must completed/provide the missing reports/inspections for the 5 year leg inspection prior to being granted credit for a Dry Dock inspections as per Policy Letter 07-02 Vsl may transit to Port Sulfur to complete the rest of the inspection. 46 CFR 177 17C

No satisfactory proof of drug/alcohol testing program. Provide records and equipment to satisfaction of CG Marine Inspector that vessel maintains a program. Reissued with code 16.

Vessel Operating Manual out of date. Vessel should have updated operating manual with simple to understand details of vessel.

No satisfactory proof of Firefighting and Man Overboard drill records. Provide record-keeping activity of sufficient fire and lifesaving drills to satisfaction of CG Marine Inspector.

No satisfactory proof of Lifesaving Equipment Maintenance Records. Provide maintenance records for lifesaving equipment onboard.

Life ring buoy requires adequate length throw line. Current line does not allow for satisfactory use while the lift boat is maintaining an air gap.

Replace floating light battery with approved type to ensure proper buoyancy and operation.

Replace retro-reflective tape with SOLAS approved type, current lifesaving equipment is using DOT traffic-type tape.

No satisfactory proof of Fire Control/Safety Plan. Provide adequate plan to CG Marine Inspector for approval.

Loose electrical fittings on switchboard/panelboard in operating house creating potential for electrical shock. Tighten fittings to satisfaction of CG Marine Inspector.

Fuel hoses on main engine are not approved type and create a potential risk of fire. Replace hoses with J1942 hoses or that of approved type for marine fuel system to satisfaction of CG Marine Inspector.

Brackets securing fuel line on main engine are broken, allowing chafing of line. Replace brackets to satisfaction of CG Marine Inspector.

Positive lead wire connecting battery to starter has exposed wire at mounting post and is making contact that provides a potential risk of shock and/or fire.

Shackles connecting steering cable to rudder have loose pins and are not secured with a cotter pin or other means, posing a potential loss of steering risk. Secure steering cables to satisfaction of CG Marine Inspector.

Wiring methods in battery box are insufficient with loose wiring and pose a risk of electrical shock and/or fire. Correct to satisfaction of CG Marine Inspector.

Flexible connections for hydraulic fluid service from tank are leaking, posing risk of an equipment malfunction while vessel is in an air gap state. Repair to satisfaction of CG Marine Inspector. Vessel relies on hydraulics for fire pump and life boat service.

Fuel tank service port has valve leaking by and no cap to prevent fuel leaking into the bilge, posing a potential risk for fire. Correct to satisfaction of CG Marine Inspector.

Vessel has permanently affixed, portable gasoline generator on deck in the immediate vicinity of a fuel tank vent. Operating manual should have instructions for fueling, use, and servicing of generator to include personnel and fire risk mitigation.
