The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. PSCO observed multiple cardboard boxes, open paint cans, and approximately 35 plastic containers containing waste oil on the 3rd deck of the engine room adjacent to fuel tanks.

The purpose of this regulation is to provide means of escape so that persons on board can safely and swiftly escape to the lifeboat and liferaft embarkation deck. For this purpose, the following functional requirements shall be met: escape routes shall be maintaned in a safe condition, clear of obstacles. PSCO observed marked escape door padlocked shut from the outside, unable to be opened from the inside, on the main deck adjacent to the pain locker.

On cargo ships, each lifejacket shall be fitted with a lifejacket light complying with the requirements of paragraph 2.2.3 of the Code. PSCO observed 02 lifejackets on the bridge did not have affixed lights.

The company and the ship shall comply with the requirements of the international safety management code. For the purpose of this regulation the requirements of this code shall be treated as mandatory. In meeting these requirements, the company should ensure that any nonconformity is reported, with its possible cause if known; appropriate corrective action is taken; and records of these activities are maintained (10.2). The deficiencies observed by the PSCO and noted in #2, #3, and #4 offer objective evidence that the vessel is neither being maintained nor is in substantial compliance with the relevant conventions, calling into question the adequacy and implementation of the vessel's SMS under the ISM code. The system shall be provided with arrangements to ensure that any discharge of oily mixtures is automatically stopped when the oil content of the effluent exceeds 15 ppm. PSCO observed three-way valve of the oily water separator not functioning properly. The valve only went overboard or completely cut off, preventing the three-way valve from recirculating. MARPOL 14.7 30AC

The purpose of this regulation is to maintain and monitor the effectiveness of the fire safety measures the ship is provided with. For this purpose, the following functional requirements shall be met: fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. PSCO requested to test two fire hoses on deck utilizing the emergency fire pump. Crew attempted to energize fire hoses while running on power from the emergency fire pump however, no stream of water was produced from either hose. 74 SOLAS (09) II-2/14.1 30AC

Means shall be provided to control leaks of flammable liquids. Vessel had an oil discharge while conducting bunker operations. Diesel oil was discharge from the STBD side Diesel Diesel oil tank through the tank vents into engine room bilge and deck containment. An unknown amount of fuel was discharged into the waterway creating a sheen alongside the vessel. 74 Solas (20) II-2/4.1.1

Ships shall be provided with arrangements, equipment and fittings of sufficient safe working load to enable the safe conduct of all towing and mooring operations. Vessel's aft mooring winch is not operational. SOLAS (2020) II-1/3.8.2

The total suction head of the Emergency Fire Pump on the pump capacity under all conditions of list and trim. Vessel's emergency fire pump was unable to obtain suction when at 4.6m draft. SOLAS (2020) II-2/10.2.2.3.1.2 / FSS CODE 12.2
