The condition of the ship and its equipment is maintained to conform with the provisions of the code to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship persons on board or the environment. Attending MI obvserved an inoperable aft towing winch resulting from an incident that occurred on 16 September 2025. - CCSSC Ch 1. / Reg 1.11.1

CCSSC (1997) 4.9.5. Every liferaft shall be stowed with its painter permanently attached to the ship and with a float-free arrangement so that the liferaft floats free and, if inflatable, inflates automatically when the ship sinks. Liferafts were incorrectly installed with the painter line attached to a shackle above the hydrostatic release instead of the weak link.

All electrical installations, equipment, cables, and wiring should be so installed and protected as to reduce to a minimum any danger to persons on board. Vessel's engine room had unsecured batteries at generators, cut and unsecured wires on stbd generator, and cut/dead-end wires on port side fire alarm activation button. 95 CCSSC 2.3

The owner or Master of every ship to which this Code applies shall ensure that the condition of the ship and its equipment is maintained to conform with the provisions of this Code to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship, persons on board or the environment. 02 smoke detectors in the engine room were not functional. CCSSC (2015) 1.11.1

Any oil or other substances flammable or harmful to the marine environment shall not be carried in other tanks or spaces which are not specially approved by the Administration for such purposes. Fuel oil was found stored in gas cans in the engine room. CCSSC (2015) 3.20.8

Each lifeboat shall be inspected weekly which shall at least consist of: visual inspections to ensure that they are ready for use. Sponson on lifeboat was deflated rendering the boat not ready for immediate use. CCSSC (2015) 4.17.7.1

The master shall ensure that the frequency of drills performed shall not exceed the following frequencies if weather and circumstances permit: Fire Drills – monthly. No records of fire drills could be provided since March 2022. CCSSC (2015) 7.4.3.1

The maintenance and operation of all the equipment and instillations on board for firefighting and containment of fire should be kept up to date. Heat detector is broken in galley. CCSSC 97 3.13.3

An Administration may, if circumstances require, subject to the provisions of paragraph 1, allow a seafarer to serve for a period not exceeding three months on board a ship entitled to fly its flag, while holding an appropriate and valid certificate issued and endorsed as required by another party for use on board that party' ships but which has not yet been endorsed so as to render it appropriate for service on board ships entitled to fly the flag of the Administration. Documentary proof shall be readily available that the application for an endorsement has been submitted to the Administration. Chief Engineer and Second Engineer began working on board prior to flag receiving their application for endorsement. STCW Ch. I/10.5

Deficiencies which may be deemed to pose a danger to persons, property or the environment include the failure of seafarers to hold a certificate or to provide documentary proof that an application for an endorsement has been submitted to the Administration in accordance with regulation I/10, paragraph 5. STCW I/4.2.1 Coast Guard inspectors were made aware upon arrival that the master and chief engineer both did not possess a flag state endorsement. After review, it was found that the master's contract began on board the 15th of March, 2020 and the chief engineer's contract began on board the 19th of December, 2019. Both the master and chief engineer have lacked a valid flag state endorsement since arriving on board.

Each Administration shall hold companies responsible for the assignment of seafarers for service on their ships and shall require every such company to ensure that each seafarer holds an appropriate certificate. STCW I/14.1 The company failed to ensure its crew was properly licensed.

Every fire hose shall be provided with an approved nozzle and the necessary couplings. PSCO observed fire hose on STBD side of the engine room with no nozzle.

Fire control plans shall be kept up to date. PSCO observed outdated fire control and safety plans.

Rescue boat shall be stowed in a state of continuous readiness for launching in not more than five minutes. PSCO observed rescue boat would not start within the required time limit.

Electrical installations shall be such that the safety of personnel and ship from electrical hazards will be assured. PSCO observed multiple electrical shock hazards. Dead end wires in Machinery space, exposed open light fixtures in crew cabins and multiple broken light switches on vessel.

Clear instructions to be followed in the event of an emergency shall be provided and exhibited in conspicuous places throughout the ship and shall specify details of the emergency alarm and action to be taken by crew or other persons on board. PSCO observed multiple versions of the emergency instructions (Station Bill) including an outdated version.

Training drills shall as far as practicable be conducted as if there were an actual emergency. PSCO observed two unsatisfactory fire drills, slow reaction, crew not familiar with fire billet, lack of communication, lack of knowledge on firefighting procedures and unfamiliarity with firefighting gear.

Each survival craft and rescue boat shall be marked in accordance with the requirements of the LSA code as applicable. PSCO observed rescue boat without required markings.

0999- The owner or master of every ship to which this Code applies should ensure that: whenever an accident occurs to a ship, of which affects the safety of the ship or the efficiency or completeness of its life-saving appliances or other equipment, a request should be made immediately to the Administration responsible for issuing the relevant certificate for a survey as may be required by 1.10, be carried out as soon as practicable. Received report that M/V SENTRY allided with aid to navigation CANTINA PORT BASIN #15, buoy 33200 on 30 July 2018. Report stated that some chain from buoy wrapped itself around the shaft and 02 of the propeller blades. - CCSSC (2002) Ch.1/11.3 - 60ac

0615 - Rescue boats shall be launched each month with assigned crew. Vessels rescue boat has not been launched since 26MAR18 - CCSSC (2015) CH.4.17.4 - 15c

0750 - Emergency fire pump shall be positioned in safe place outside engine room and permanently connected to fire mains. Pump is permanently connected to the fire main - CCSSC (2015) CH 3.3.10.1 - 50ac

1730 - The condition of the ship and its equipment is maintained to this code to ensure that the ship will remain fit to proceed to sea without danger to ship, persons on board and the environment. In vessel engine room a OWS is installed. This piece of equipment is not listed on the CCSSC. Due to vessel gross tonnage OWS is not required and must be removed - CCSSC (2015) CH 1.11.1 - 50ac

In every ship, there shall be provided for the protection of any space containing internal combustion type machinery having a total power output of 750kW and above: one of the fixed fire-extinguishing systems referred to in 3.10.1. The total machinery space kilowattage is 2,388 and no fixed extinguishing system is installed. CCSSC 3.10.3

Every seafarer holding a certificate issued under the provisions of the convention, who is serving at sea, shall also hold a valid medical certificate issued in accordance with the provisions of this regulation and of A.1/9 of the STCW Code. Two personnel assigned to the vessel do not have valid medical certificates. STCW Reg. I/9.3

Life saving appliances required by this chapter shall comply with the technical specifications of the LSA Code. The rescue boat does not contain equipment required by the LSA Code. CCSSC 4.3.1, LSA Code 5.1.2.2

The ship in all respects will remain fit to proceed to sea without danger to the ship, persons onboard, or the environment. There was an exhaust leak in the engine room on the starboard main engine. CCSSC 1.11.1

Nautical charts and nautical publications such as sailing directions, list of lights, notices to mariners, tide tables, and all other nautical publications necessary for the intended voyage, shall be adequate and up to date. The vessel is not equipped with up to date current tables, coast pilot, and inland navigation rules. 74 SOLAS (2014), Ch. V / Reg. 27

Masters shall ensure that the frequency of drills performed shall meet the following frequencies: enclosed spaces and steering gear - quarterly. The crew have not performed the required drills.

In all such ships, fire control plans shall be kept up to date. The current fire control plan lists one fire fighting outfit. There are two fire fighting outfits onboard. Additional fire fighting equipment was observed in the engine room and not found in the current fire control plan. CCSSC 3.13.2

A daylight signalling lamp, or other means, to communicate by light during day and night using an energy source of electrical power not solely dependent upon the ship's power supply is required on all ships of 150 gross tonnage and upwards. There is no independent source of electrical power that meets the requirements of IMO Resolution MSC.95(72). 74 SOLAS (2014), Ch. V / Reg. 19.2.2.2

All ships shall carry an up-to-date copy of Vol. III of the International Aeronautical and Maritime Search and Rescue (IAMSAR) Manual. The vessel is not equipped with an up-to-date IAMSAR Manual. 74 SOLAS (2014), Ch. V / Reg. 21.2

The person in charge of any vessel equipped with ballast water tanks must maintain a ballast water management (BWM) plan that has been developed specifically for the vessel. The vessel does not have a ballast water plan. 33 CFR 151.2050(g)

This permit is effective from December 19, 2013 to December 18, 2018 (i.e. 5 years). It provides NPDES permit coverage nationwide for discharges incidental to the normal operation of commercial vessels greater than 79 feet in length. The vessel does not have a Vessel General Permit.

Unless otherwise expressly provided, this Code shall apply to cargo ships of less than 500 GT incl. oil tankers & tankers, irrespective of length, engaged on int'l voyages trading in the Caribbean Trading Area. The vessel's GT is 290 & its primary operating areas is in the Caribbean Trading Area, incl. U.S. waters; however, the vessel does not have a valid Caribbean Cargo Ship Safety Certificate.

0230- Al ships shall be sufficiently and efficiently manned, Vessel did not have on board one deck officer (II/1), which is required under the issued Minimum Safe Manning document. Solas 2009 V/14.1 17C

the certificates issued under 1.12 should be readily available on board for examination at all times. Load line, solas equip, solas construction, solas radio and caribbean cargo ship safety code cert on board were expired. CCSSC ch 1 part B 1.15 due to USCG

1730 - The condition of the ship and its equipment is maintained to conform with the provisions of this code. The Oil Water Separator has been inoperable for the last six months. No conditions of class on board. CCSSC Code 1.11.1 - 50ac

A/B found not on board, so the vessel does not meet safe manning requirements. Deficiency written to be corrected prior to departure from port.

Oily water separator does not function, but is not required. Vessel's owner stated that the OWS will be removed or put out of service.

CCSSC 2.3 VESSELS MAIN ENGINES SHALL BE OPERABLE FOR THE SERVICE INTENDED. PORT MAIN ENGINE FOUND NOT OPERATIONAL.

CCSSC 5.2.3.3= EPIRB SHALL BE CAPABLE OF FLOATING FREE IF THE SHIP SINKS. HYDROSTATIC RELEASE MECHANISM FOR EPIRB FOUND EXPIRED.

CCSSC 1.11= THE OWNER OR MASTER OF EVERY SHIP TO WHICH THIS CODE APPLIES SHALL ENSURE THE CONDITION OF THE SHIPAND ITS EQUIPMENT IS MAINTAINED TO CONFORM WITH PROVISIONS OF THIS CODE. ONE LIFE BOUY ON THE PORT SIDE FOUND WASTED.

FIXED FIRE DETECTION SYSTEM AND ALARM SYSTEM SHALL COMPLY WITH THE REQUIREMENTS OF SOLAS. 02 SMOKE DETECTORS IN ENGINE ROOM FOUND INOPERABLE. IN ADDITION, CREW MEMBER CABINS ARE MISSING SMOKE DETECTORS.

CCSSC 4.9.8.1= RESCUE BOAT SHALL BE IN A STATE OF CONTINUOUS READINESS IN NOT MORE THAN 5 MINUTES. RESCUE BOAT ENGINE FOUND INOPERABLE.

CCSSC 4.17.2= TRAINING DRILLS SHOULD AS FAR A PRACTIABLE BE CONDUCTED AS IF THERE WERE AN ACTUAL EMERGENCY. WHEN CONDUCTING FIRE DRILL, CREW DID NOT SEEM TO BE KNOWLEDGABLE IN HOW TO EFFICIENTLY DON FIRFIGHTING GEAR AND APPROACH THE FIRE. AFTER CONDUCTING 02 FIRE DRILLS, IT WAS DETERMINED THAT TRAINING SHALL BE CONDUCTED PRIOR TO DEPARTURE.

40CFR122.2= ALL VESSELS OF 79 FEET OR GREATER IN LENGTH SHALL HAVE ONBOARD A VESSEL GENERAL PERMIT. THE VESSEL OWNER STATED THAT THEY WERE NOT AWARE OF THESE REGULATIONS AND THEREFORE DO NOT HAVE A VESSEL GENERAL PERMIT AT THIS TIME.

CCSSC 4.1.2= AT LEAST TWO OF THE LIFEBOUYS SHALL BE PROVIDED WITH SELF ACTIVATING SMOKE SIGNALS. SELF ACTIVATING SMOKE SIGNAL ON PORT SIDE FOUND BROKEN.

Electrical Installation should be such that the safety of personnel and ship from electrical hazards will be assured. Electrical switchboard that services port and starboard ship service generators does not have non-conducting mating in front of it. Install mating to the satisfaction of attending Port State Control Officer. CCSS Code 2.26

The strength and construction of superstructures should be sufficient to withstand all foreseeable conditions of the intended service. Exhaust port for ventilating crew bathroom has rusted/deteriorated screen. Replace screen to the satisfaction of attending Port State Control Officer. CCSS Code 2.4
