The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules (incorporated by reference, see § 58.03-1), except as otherwise provided by this subchapter. Vessels remote engine control became inoperable during transit. Rectify remote engine control failure to the satisfaction of class. The arrangements provided shall be such as to ensure the safety of the ship in all sailing conditions is equivalent to that of a ship having the machinery space manned. MI received notification that the vessel's fire detection system is not properly functioning. Vessel is required to maintain a 24 hour fully manned engine room and conduct additional rounds in all areas where the detection system is inoperable until the detection system is operable and system functionality has been verified.

The ring, lines and letters shall be painted in white or yellow on a dark ground or in black on a light ground. They shall also be permanently marked on the sides of the ships to the satisfaction of the Administration. The marks shall be plainly visible and, if necessary, special arrangements shall be made for this purpose. MI found STBD plimsol mark worn and illegible.

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: .1 structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions. MI found door between galley and crew messroom would not close & door between crew messroom and passageway would not close. 74 SOLAS (20) II-2/14.2.1.1

The arrangements of the ship's main source of electrical power shall be such that the services referred to in regulation 40.1.1 can be maintained regardless of the speed and direction of rotatation of the propulsion machinery or shafting. While underway, vessel experienced loss of power to #1 auxiliary generato, which casused temporary loss of propulsion.

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power required by regulations II-1/42 or II-1/43 as appropriate. Indentified two emergency lights at the rescue boat and life rafts that are inoperable on starboard side. Port side emergency light was half full of water at life rafts. SOLAS 14 III/11.4

On each tanker of 10,000 GRT or more that is subject to 46 U.S.C. 3708, the dual radar system required by this part must have a short range capability and a long range capability; and each radar must have a true north feature consisting of a display that is stabilized in azimuth. Vessels S Band radar is inop. Make repairs to s band radar and provide tech report to Sector LA/LB OCMI prior to departure.

The Company should establish procedures, plans, and instructions for key operations concerning the safety of the ship. The various tasks should be defined and assigned to qualified personnel. Make available for USCG review, from the vessel's Safety Management System (SMS), the procedures for key operations and assignment of qualified personnel to stand a full engineering watch meeting STCW and 46 USC 8104 requirements, in the event of a vessel's loss of vital system automation, including but not limited to; control systems, alternate control systems, safety control systems, alarms and instrumentation, programmable systems and devices, and all applicable systems required to maintain the ACCU notation on your ABS Classification Document. The vessel's SMS should provide for measures ensuring that the Company's organization can respond at any time to emergency situations involving the ship. Make available for USCG review, from the vessel's SMS, established and approved processes in place to ensure the vessel's crew can safely respond to and maintain adequate safety measures in the event of a vessel's loss of vital system automation, including but not limited to; control systems, alternate control systems, safety control systems, alarms and instrumentation, programmable systems and devices, and all applicable systems required to maintain the ACCU notation on your ABS Classification Document. The manning requirements for a particular vessel are determined by the OCMI after consideration of the applicable laws, the regulations in this part, and all other factors. Members of the maintenance department do not meet requirements from applicable USCG guidance (i.e. PUMS endorsement requires at least 2 "qualified personnel" per NVIC 1-69, referred from MSM Volume II, Section B, Chapter 1.

Prove proper operation on hydralic lock alarms on steering gear IAW PSTP

Make shower and eye wash fountains operational so they are available at all times.

Provide warning signs for transfer operations

Make load line rings, lines, and letters plainly visible.

Repair or replace knife edge on forward escape hatch.

Repair or replace the defuezer on the lifeboat forward escape hatch.

Make repairs to #1 compressor. 46 CFR 62.35-35 Prior to departing Port Valdez. Shifting of berths is authorized if necessary.
