A towing vessel subject to this section must maintain a Towing Vessel Record (TVR)… The TVR must include a chronological record of events in addition to the items to be recorded in accordance with 46 CFR 140.915(b). A record review of the vessel’s TVR from October 2023 to March 2024 indicated that the vessel has not recorded events and required items in accordance 46 CFR Subchapter M. For instance, the vessel’s TVR chronological record of events was inconsistent and often left blank without record of vessel movements and operations. Personnel records were incomplete, not listing names and positions while operating the vessel as required. Navigation assessments were not being fully completed and documented as required. Rigid Constructors shall ensure mariners operating the ITV ANGELA MARIE are familiar with TVR requirements and have the resources to complete adequate records and navigation assessment prior to getting underway, upon watch changes, and as conditions change during the voyage/operation.

Essential systems or equipment must be regularly tested & examined. Tests & examinations must verify that the system or equipment functions as designed. If a component is found unsatisfactory, it must be repaired or replaced. During the steering gear test, it was observed that the hydraulic hoses on the rudder control system was wasted, bulging & rubbing against a wasted metal. The vessel intends to replace all hoses, crop & renew the wasted metal & add chafing gear to the hoses.

o person may intentionally drain oil or hazmat into the bilge of a towing vessel from any source. During the inspection it was observed that oil from the main diesel engine was intentionally drained into the bilge. The vessel personnel intend to clean bilge & provide objective evidence.

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. Identified plastic sight glass in engine room; not contained.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Identified water hose severly chaffed due to raw water belt.

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. STBD void gasket inoperable.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Loose wiring from generator to distribution panel.

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space and must be either: (1) Automatic, battery-operated with a duration of no less than 2 hours; or (2) Non-electric, phosphorescent adhesive lighting strips that are installed along escape routes and sufficiently visible to enable egress with no power. E/R emergency light lower deck inoperable.

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. Day shapes no on-board.

Survival craft must be stowed in accordance with the requirements of § 199.130 of this chapter, as far as is practicable on existing towing vessels. Break away missing tag indicating approval number.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency operations. The vessel's port generator is not functioning. Vessel is restricted to inner harbor movement and operations. Install new generator within 14 days.

When the towing vessel is underway, the EPIRB must be stowed in its float-free bracket for automatic activation and be mounted in a manner so that it will float free if the towing vessel sinks. Hydrostatic release unit cannot be confirmed as not expired. Record of installation and expiration necessary to maintain current unit or replace 46 CFR 140.415

Prior to getting underway for the first time on a towing vessel, each crewmember must receive a safety orientation. No record of orientation for any crewmembers 46 CFR 140.410

Storage areas are kept free from accumulation of combustible and flammable materials insofar as practicable. Charcoal briquettes stored in engine room. 46 CFR 142.220

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vesel to determine the presence of alcohol. Testing strips were expired by more than 7 months. 46 CFR 4.06-15

attending inspector observed severe oil leak from rear seal on port generator resulting in pooled oil beneath generator. In addition, there was used oil, fuel and old batteries being stored in the engine space
