The condition of the ship and its equipment shall be maintained to ensure ship will remain fit to proceed to sea. While vessel was transiting outbound Houston Ship Channel in vicinity of Galveston Jetty Buoy 1B, vessel collided with tug and barge, causing damage to starboard forward quarter resulting in a 12 inch by 1 inch tear inside shell plating in way of frame 136-135..
