When testing the remote fan shutdowns for the engine room, the #2 generator shut down and dropped the load from the main switchboard causing a temporary blackout. 46 CFR 111.10-4(d)

Resolution: UPS now working properly. Verified proper labeling of emergency shutdowns for fuel oil and lube oil pumps/seperator. The UPS charger is now added to panel L-6 as a seperate breaker. Previously it was wired in to the remote shut downs for the engine room.

Each OSV must have posters or signs displayed in the vicinity of each survival craft and the survival craft's launching controls that— illustrate the procedures for operating the launching device. The liferaft launching instructions posted do not accurately depict the launching procedures to be followed in an emergency. If followed, the painter would not be secured, allowing the liferaft to be deployed without inflating the rafts. 46 CFR 133.90(b)

Resolution: Life raft stowage has been changed to allow releasing of the straps holding the life rafts in place without loosing access to the painter in accordance with launching diagram.

The vessel has a portable accommodations module (PAM) installed on the back deck of the vessel as defined in CG-ENG Policy Letter No. 01-16 paragraph 5.a.i. The PAM on board has not been certified in accordance with this policy. Correct deficiency prior to use of this portable accommodation. 46 CFR 127.120

Resolution: The Portable Accommodation Module (PAM) containing the AUV has been removed. Discussion was had on how to add a PAM in the future. We also discussed the history of this AUV and ways to move forward.

Each penetration of a bulkhead in a machinery space by piping must meet the design requirements for material and pressure in subchapter F of this chapter. Non-metallic piping penetrating the aft engine room bulkhead did not have a valve on either side of the bulkhead to make the bulkhead watertight. 46 CFR 174.195(d)

Resolution: The non-metallic piping was removed from the bulkhead and capped off. The crew also installed a metallic pipe with a cap on one side in case anyone were to install that non-metallic pipe in the future.

Scuppers and discharge pipes originating at any level and penetrating the shell either more than 171/2 inches below the freeboard deck or less than 231/2 inches above the summer load waterline shall be provided with a nonreturn valve at the shell. The non return valve in way of the side shell for the water making system is leaking. 46 CFR 42.15-60(c)
