PSCO OBSERVED OIL SOAKED INSULATION COVERING SIX FEET OF PIPE LOCATED ON LOWEST LEVEL ENGINE ROOM, PORT SIDE, FORWARD, NEXT TO ESCAPE TRUNK. ALSO OBSERVED OIL SOAKED INSULATION UNDER FUEL PUMPS IN PURIFIER ROOM FORWARD.

PSCO observed multiple electrical connections throughout the engine room with gromets or electric puddy missing on stuffing tubes. Also observed old VDR system on bridge was left in place and electrical connections were not secured or removed in effect to avoid possible shock.
