Lifeboat and rescue boat release gear must be subjected to a thorough examination by properly trained personnel familiar with the system at each inspection for certification. Marine inspector observed no annual servicing in accordance with manufacturers instructions. 46CFR199.190(i)(4)

Each inflated rescue boat must be repaired and maintained in accordance with the manufacturer's instructions. All repairs to inflated chambers must be made at a servicing facility approved by the Commandant, except for emergency repairs carried out on board the vessel. Marine inspector observed two repairs made to inflatable chamber, one of which in very poor condition and unable to hold pressure. Additionally, no annual servicing has been preformed IAW 46 CFR 199.190. 46CFR199.190(g)(4)

Each davit, winch, fall, and other launching appliance must be thoroughly inspected and repaired, as needed, once each year. Marine inspector observed no annual servicing in accordance with manufacturers instructions. Vessel shall have properly trained personnel attend for proper servicing of subject lifesaving equipment. 46CFR199.190(f)(2)

Each firehose on each hydrant must have a combination solid stream and water spray firehose nozzle that meets the requirements in 46 CFR 162.027. Firehose nozzles previously approved under subpart 162.027 of this chapter may be retained so long as they are maintained in good condition to the satisfaction of the Officer in Charge, Marine Inspection. Marine Inspector observed leak in starboard forward main deck fire hose. Vessel shall replace. 46CFR76.10-10(j)

Additional lifejackets for persons on watch must be stowed on the bridge, engine control room or other manned watch stations. No crew lifejackets were stowed on the bridge. 46 CFR 70.28-1 46 CFR 199.(b)(2)

Stowage. Lifejackets must be readily accessible. 37 passenger passenger lifejackets were stowed in the crew lounge marked "restricted area." Move the lifejackets to accessible location or ensure lounge is accessible to passengers. Limited to 112 passengers. 46 CFR 70.28-1 46 CFR 199.(b)(2)

The weatherdeck on a flush deck vessel must be made watertight. Vessel had 4 scuttles on the car/main deck that are in need of new gaskets and one with a broken dogging arm. Repair/replace.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel self-reported failed cutlass bearing on shaft for central main engine. Vessel is permitted to transit to dry dock and all repairs are to be made to the satisfaction of the CG marine inspector.

Vessel is required to service their fixed CO2 System annually. Vessel has a third party conduct the service and did not weigh and record the amount of Co2 in each bottle properly. Vessel shall re-weigh the Co2 bottles and record weights in the presence of a USCG Marine Inspector.

Vessel is required to have a FCC station license. Vessel FCC license expired June 4, 2023. Provide valid FCC station license.

Vessel is required to have up to date charts. Vessel charts were not updated. Vessel shall provide updated charts.

Each hydrostatic release unit, other than a disposable unit must be serviced in according with part 160, subpart 160.062 of this subpart. Hydrostatic release units for 3 of 4 liferafts were expired and need replaced. 46 CFR 199.190(h)

All enclosed spaces within the vessel shall be properly vented or ventilated and means shall be provided to close off all vents. New gaskets for ventilation closures for machinery space prevented closure of vents. 46 CFR 72.15-15

All automatically or remotely controlled or monitored vital systems addressed by part 62 of this subchapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features, and interlocks. Test procedures must be submitted to the Coast Guard for approval. Last approved revision (REV.B CTI-48-ALS) has since been updated to revision MAS-1248 dated January 12, 2022. New revisions must be submitted/approved by the Coast Guard (MSC). 46 CFR 61.40-1

Spaces which are protected by a carbon dioxide extinguishing system shall be fitted with an approved audible alarm, and shall be marked as required by 78.47-9 of this subchapter. "When alarm sounds vacate at once. Carbon dioxide or clean agent being released" Vessel missing required marking/signage near alarm of protected space. 46 CFR 76.15-30

Vessel was required to have a successful quarterly inspection NLT 23SEP2021. Quarterly was not conducted, vessel is restricted from carrying passengers until a satisfactory quartlery is completed.

Vessel is required to weight test rescue boat davit whenever falls are renewed. While conducting davit weight test (dynamic) the welds on the pedestal and foundation failed by way of cracking. Provide repair proposal and re-test IAW 46 CFR Subchapter W.

Vessel is required to carry IBA's to accommodate a total of 150 people. IBAs's were sent of for servicing and all but one were returned. Current configuration will only allow the carriage of 130 total persons on board. Vessel is restricted to carrying 126 passengers and four crew until they receive the remaining IBA.

Remove deadend cable in Forpeak Void

Replace green and white wire in Forpeak Void with shielded marine type cabling

Excessive shaft leakage. Tighted or repack the Port shaft.

Properly secure all scuttle covers to prevent loosing overboard

Ensure all VHF/UHF radios have the DSC switch patched in to GPS.

Replace degraded cabling for navigational lighting

Install circuit breaker blank inside the panelboard on the Bridge.

Properly secure the cable for the E/R fan located on the main deck

Replace expired batteries for all buoy lights

Fire boundaries using self-closing fire doors shall be maintained at all times. The A-15 fire door from the vestibule top teh engine room was wasted and repaired. The repair was approved as a temporary repair unitl a new door can be manufactured and delivered (8 weeks). The repairs were tested and found to be acceptable unitl the new door can be installed and tested.

Fire boundaries between the engine room and vestibule shall be maintained at all times. The A-15 self-closing door between the engien room and vestibule is wasted beyond repair. Replace the A-15 self-closing door between the engien room and vestibule.

Damaged cylinder head on the port MDE; required to make repairs prior to carrying passengers.

Limit switch on rescue boat davit winch is not properly calibrated to safely stop the winch from parting the wire.

Keel cooler piping to all 03 MDEs were missing proper insulation to protect crew from hazard.

Port Eng Room Bilge pipe had a hole drilled into it while securing deck plating. Make repairs to the port engine room bilge pipe and make all bilge pipe available for inspection.

Submit periodic safety test procedures to local OCMI for approval, IAW 46 CFR 61.40-1.

Provide a low oil level alarm for both steering hydraulic tanks, IAW 46 CFR 58.25-25.

Secure the toolbox in the engine room, IAW 46 CFR 50.20-35.

Prove operation of the STBD MDE low clutch oil pressure alarm, IAW 46 CFR 62.25-20.

Properly install the lighting fixture over the STBD MSD, IAW 46 CFR 111.75-20.

Crop and renew the return hydraulic pipe on the stern hydraulic pump, IAW 46 CFR 58.25-20.

Provide oil temp gauges for all propulsion diesel engines, IAW 46 CFR 62.35-30.

The required clearance in 46 CFR 78.40-1(a) needs to be provided for passenger safety and to provide access to fire stations, vent closures and E/R door while underway.
