A record of the age of the foam concentrates and of subsequent controls should be kept on board. MSC/Circ. 582
PSCO observed the foam test report on board reflecting 03MAR2023 as the last report. Vessel is past its annual due date for foam analysis and has no record of a new report. PSCO could not determine the reliability of the high and low expansion foam.

Oil fuel pipes, which, if damaged, would allow oil to escape from a storage, settling, or daily service tank having a capacity of 500 L and above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of bein closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated.
PSCO observed inoperable remotely operated quick closing valve on generator #3. Valve was found in the open position prior to testing.

A secondary means of allowing full flow relief of vapour shall be provided. Alternatively, pressure sensors may be fitted in each tank protected by the arrangement in paragraph 6.1.2. Such monitoring equipment shall also provide an alarm facility which is activated by detection of over-pressure or under-pressure within the tank.
Starboard Slop Tank Radar is inoperable. Part Requisition submitted 19JUL2023. Replacement status unavailable.

Resolution: Zenner barrier and MIF-SB3 card were replaced with new spares, slop STBD tank radar monitoring system. ABS conducted remote survey clearing the deficiency.

Each incinerator on a ship constructed on or after 01 JAN 2000.. each incinerator subject to this paragraph shall be approved by the Administration taking into account the standard specification for shipboard incinerators developed by the organization.
PSCO observed substantial refractory degredation inside teh incinerator, presenting a hazard to the ship and crew.
