All electrical apparatus shall be so constructed & so installed as to not casue injury when handled or touched in the normal manner. PSCO observed one overhead light in one of the walkways between the cargo holds that was missing its cover and had exposed wires, in addition another light on the starboard side had a grounding wire that was disconnected. Exposed wires were located in the walkway between bays 30 and 34. SOLAS 74(04) CH. II-1/45.1.3

IN ADDITION, IT SHALL BE PROVIDED WITH ALARM ARRANGEMENTS TO INDICATE WHEN THIS LEVEL CANNOT BE MAINTAINED. THE SYSTEM SHALL ALSO BE PROVIDED WITH ARRANGEMENTS TO ENSURE THAT ANY DISCHARGE OF OILY MIXTURES IS AUTOMATICALLY STOPPED WHEN THE OIL CONTENT OF THE EFFLUENT EXCEEDS 15 PPM. IN CONSIDERING THE DESIGN OF SUCH EQUIPMENT AND APPROVALS, THE ADMINISTRATION SHALL HAVE REGARD TO THE SPECIFICATION RECOMMENDED BY THE ORGANIZATION. PSCO OBSERVED FAILURE OF AUTOMATIC STOP WHEN EFFLUENT EXCEEDED 15PPM, AS WELL AS NO AUDIBLE OR VISUAL ALARM IN THE SPACE. MARPOL 1/.14.7 16AC

PSCO observed wasted handhold lines on the port side4 of the port lifeboat.
