Each ship subject to the Bridge-to-Bridge Act must have on board a radio operator who holds a restricted radiotelephone operator permit or higher class license. Vessel master did not provide proof of current radiotelephone operator permit. 47 CFR 80.163

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. Vessel did not have regular liferaft onboard at the time of inspection. 46 CFR 141.230

The owner of a vessel subject to inspection under section 3301 shall- (1) ensure that such vessel is equipped with a vessel master key control system, manual or electronic, which provides controlled access to all copies of the vessel's master key of which access shall only be available to the individuals described in paragraph (2); (2) establish a list of all crew, identified by position, allowed to access and use the master key and maintain such list upon the vessel, within owner records and included in the vessel safety management system; (3) record in a log book information on all access and use of the vessel's master key, including- (A) dates and times of access; (B) the room or location accessed; and (C) the name and rank of the crew member that used the master key; and(4) make the list under paragraph (2) and the log book under paragraph (3) available upon request to any agent of the Federal Bureau of Investigation, any member of the Coast Guard, and any law enforcement officer performing official duties in the course and scope of an investigation. Vsl has no Master Key, identified crew, or logbook.

Each towing vessel operating in those regions must carry at least one appropriate-size immersion suit, approved under approval series 160.171, for each person on board. Vessel is equipped with 05 of 06 required immersion suits that are not appropriate size for crew, VSL to replace 02 Jumbo sized with adult universal sized.

The master of a towing vessel must ensure that drills are conducted and instructions are given to ensure that all crew members are capable of performing duties expected of them during emergencies. MI observed that no man overboard drill or abandon ship drill had been conducted within the last year. 46 CFR 140.420(a)

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage. On board paper charts were 3-9 years old and without updates. Electronic charting system had not been updated since May 2022. 46 CFR 140.705(b)

Electrical components that pose an electrical hazard must be in an enclosure. CGMI observed exposed wire coming from a junction box on the bridge. 46 CFR 143.400(g)

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. CGMI observed missing blank covers on port side breaker panel #1 in the engine room. 46 CFR 143.400(a)

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. Repair wasted fwd hatch and replace gasket. 46 CFR 140.610(b)

The owner or managing operator must implement procedures to identify and mitigate health and safety hazards. Crop and renew wastage hole IWO port side stairs bridge deck with at least 18" x 18" insert plate. 46 CFR 140.510(a)

Towing vessels must minimize fire hazards. The fuel transfer system located on the forward bulkhead of the engine room has three small leaks in the fixed piping. Repair leaks to the satisfaction of the attending Coast Guard Marine Inspector.

Means must be provided for closing ventilation in case of a fire. Vessel must provide means to secure ventilation to the engine room from the mushroom vent on the top deck.
