The bunker delivery note shall be accompanied by a representative sample of the fuel oil delivered taking into account resolution MEPC.182(59). The sample is to be sealed and signed by the supplier's representative and the master or officer in charge of the bunker operation on completion of bunkering operations and retained under the ship's control until the fuel oil is substantially consumed, but in any case for a period of not less than 12 months from the time of delivery. Signed and Sealed Samples of fuel oil delivered to the vessel were not maintained.MARPOL (2022) Annex VI/18.8.1

Measures shall be taken to the satisfaction of the Administration to ensure that the equipment is functioning in a reliable manner and that satisfactory arrangements are made for regular inspections and routine tests to ensure continuous reliable operation. Every ship shall be provided with documentary evidence, to the satisfaction of the Administration, of its fitness to operate with periodically unattended machinery spaces. The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the Company. In meeting these requirements, the Company should ensure that: inspections are held at appropriate intervals. Routine tests, testing procedures and documentation of testing completion is not captured in the SMS. Testing SSDG’s and other equipment required to demonstrate reliable operation of an an unmanned engine room has not been established and therefore all testing required is not being completed. 74 SOLAS (2020) II-1/46.2 and 46.3 74 SOLAS (2020)IX/3.1 ISM Code A/10.1

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction for the service for which they are intended. The company and the ship shall comply with the requirements of the International Safety Management Code. The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the Company. In meeting these requirements, the Company should ensure that: inspections are held at appropriate intervals; any non-conformity is reported with its possible cause, if known; appropriate corrective action is taken; and records of these activities are maintained. Gauges throughout the vessel have not been maintained. Gauge maintenance and calibration is not implemented in the SMS and therefore is not being completed.74SOLAS(2020) II-1/26.174 SOLAS (2020)IX/3.1 ISM A/10.1

Vessel reported a partial loss of propulsion. Main Engine governor alarm only allows the main engine to be controlled locally from the engine room instead of the bridge. Proper operation of propulsion system should be demonstrated from local and bridge control to an attending Coast Guard marine inspector or attending classification society surveyor with a report submitted to our office. Vessel is required to carry two additional licensed engineers so as to maintain a fully manned engine room at all times. 74 SOLAS (20) I/11(a)

Machinery essential for the propulsion of the ship shall be provided with effective means for its operation. Vessel experienced a critical slowdown override on the main propulsion engine due to malfunctioning exhaust valves on cylinders 2, 4, & 5 and was forced to return to anchorage.

Each cargo tank of a tankship must be equipped with an intrinsically safe high-level alarm and a tank overfill alarm; The tank overfill alarm required by paragraph (a) of this section must activate an alarm early enough to allow the person in charge of transfer operations to stop the cargo transfer before the tank overflows. STBD slop tank overfill alarm stuck in alarm mode. Tank shall not be used for cargo until alarm is proven effective. SOLAS 20 II-2/11.6.3.1 46 CFR § 39.2007(d)(4)

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Port Lifeboat: Install Davit data plate. Starboard Lifeboat: Repair/ replace damaged seat belt, Install Davit data plate. Vessel is limited to 30 POB until seat belt is replaced. SOLAS 20 III/20.2

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. 440v junction boxes for STBD E/R supply fans are wasted. SOLAS 20 II-1/45.1.3

The hospital must have a toilet, washbasin, and bathtub or shower conveniently situated. Other necessary suitable equipment such as a clothes locker, a table, and a seat must be provided. All washbasins, showers, and bathtubs must be equipped with adequate plumbing, including hot & cold running water. All toilets must be installed with adequate plumbing for flushing. Hospital space washbasin unable to provide hot & cold water. 46 CFR § 32.40-35(d) 46 CFR § 32.40-25(c)

All automatic, remotely controlled, or monitored vital systems addressed in 46 CFR Part 62 must have approved Periodic Safety Test Procedures(PSTP) available onboard. Provide approved PSTPs that meet the requirements of 46 CFR 62. US ACP Single Supplement (2021) 3.1.1.4

Manufacturers intending to fabricate pressure vessels, heat exchangers, evaporators, and similar appurtenances, covered by the regulations in this part shall submit detailed plans in accordance with subpart 50.20 of this subchapter. Provide Class certification that the pressure vessels were built to a Code or standard. 46 CFR § 32.35-1 46 CFR § 54.01-18(a) NVIC 10-81

One or more pressure/vacuum-breaking devices shall be provided. Repair/Replace sight glass plunger valve on PV Breaker. SOLAS 20 II-2/11.6.3.4

Products marked with an approval number containing one of the following Coast Guard approval series must be used on vessels enrolled in the ACP: 162.039 Semi-Portable Marine Type Fire Ext; Fire extinguishers shall be of a type & number required by the applicable regulations & shall be Coast Guard / UL approved. Engine casing semi-portable extinguisher is not UL approved. US ACP Single Supplement (2021) 3.3.1 NVIC 10-81, CH-1 46 CFR 34.50

Each navigation light must meet the following: Be certified by an independent laboratory to the requirements of ANSI/UL 1104 or EN 14744 or an equivalent standard under § 110.20–1 of this subchapter. Navigation lights were not UL 1104 approved. 46 CFR § 111.75-17(d)(2)

Satisfactory means (in the form of guard rails) shall be provided for the protection of the crew in getting to and from their quarters, the machinery space, and all other parts used in the necessary work of the ship. Railing on the raised walkway to the #6 STBD PV riser was found wasted. ICLL 25(4)

General arrangement plans shall be permanently exhibited for the guidance of the ship's officers. Fire control & safety plan requires approved updates for new lifesaving, firefighting equipment and missing smoke detectors. SOLAS 20 II-2/15.2.4.1

Engines with power output of more than 130 kW that are listed in this paragraph (c) must be covered by a valid EIAPP certificate, certifying the engine meets the applicable emission standards of Annex VI, unless the engine is excluded under § 1043.10 or paragraph (d) of this section. Provide US EPA issued EIAPP certificates for Main Engine and Generator Sets.

Each oil content meter component of the monitoring system installed on a U.S. vessel must be approved under 46 CFR part 162, subpart 162.050. Renew ODME equipment or provide an equivalency prior to use of equipment. 33 CFR § 157.12
