Access doors and access hatch covers normally closed at sea, intended to ensure the watertight integrity of internal openings, shall be provided with means of indication locally and on the bridge showing whether these doors or hatch covers are open or closed. A notice is to be affixed to each such door or hatch cover to the effect that it is not to be left open. Nos. 86 port and 124 port watertight doors were found with tape disabling the indicators and no. 52 starboard watertight door was found tied open with the indicator disabled by a hose section. 74 SOLAS 09 CONS. II-1 13-1.3

Valve for high side lube oil tank is leaking air.

Doors fitted in boundary bulkheads of machinery spaces of category A shall be reasonably gastight and self-closing. The entrance door to the steering gear room, and door to a storeroom within the steering gear room both had broken self-closing hinges. Vessel replaced hinges with spare parts onboard; repairs were verified by PSCO.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Vessel reported knocking in main engine scavenge unit number 4 and stopped main engine while departing Port Hueneme. Vessel reported loose bolts were found after equipment overhaul the previous day, and that bolts were able to be removed and damage to stuffing box able to be repaired. - SOLAS 74a I/11 - 16a
