The purpose of this regulation is to maintain and monitor the effectiveness of the fire safety measures the ship is provided with. For the purpose, the following functional requirement shall be met: .fire protection systems and fire-fighting systems and appliances shall be maintained ready for use; 01 CO2 fire fighting bottle was found to be empty approximate 45 days ago in CO2 Room rendering the systems not ready for its intended purpose.

All ships of 300GT and upwards shall be fitted with a 9 GHz radar. Vessel reported that the 9 GHz (X-band) radar is inoperable. SOLAS 20 V/19.2.3.1

Each Captain of the Port may order a vessel to operate in the manner directed when the Captain of the Port has determined that such order is justified in the interest of safety by reason of weather, visibility, sea conditions, temporary port congestions, other temporary hazards circumstances, or the condition of the vessel. Firefighting systems shall be kept in good working order and readily available. Multiple fire stations throughout cargo deck and main deck found leaking by while in the closed condition. 74 SOLAS (14) II-2/14.2.1.2 50A

Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazards shall be ensured. Multiple electrical enclosures in forward Bosn Locker found broken and unable to close also rendering inoperable remote reach rods for breaker closures. Forward paint locker CO2 control enclosure found broken and unable to close.

The means for securing doors shall consist of gaskets and clamping devices and be arranged that they can be operated from both sides of the bulkhead. Store room watertight hatch found unable to close due to incorrect dogging configuration. ICCL 1969 Reg 12.1 50A

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Port lifeboat engine stalled multiple times while attempting to clutch in gear box. Corrected on spot and verfified proper operation.

Main and AUX machinery essential for the propulsion and safety of the ship shall be provided with effective means for its operation and control. VSL suffered a loss of propulsion while departing Bayport Terminal, dock #4. VSL shall enact permanent repairs to the satisfaction of the RO. 74 SOLAS II-1/31.1 60ac

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Vessels Suez crane had extensive wastage on the luffing motor cable drum. Repair/replace and prove safe operation and weight handling capacity to the satisfaction of the vessels classification society.

Every lifebuoy shall, if intended to operate the quick release arrangement provided for the self-activated smoke signals and self-igniting lights, have a mass of not less than 4 kg. Lifebuoy's on port and stbd side of bridge wings were incorrectly placed and not suitable for quick release from the bridge.

The launching mechanism shall be so arranged that it may be actuated by one person from a position within the survival craft. During lowering of the Port side lifeboat, inspectors noted that the remote release cable was too long to be pulled form the inside the lifeboat.

The means of control of any fixed gas fire-extingushing system shall be simple to operate and at each location there shall be clear instructions relating to the operation of the systen(FSS 5/2.1.3.1). During inspection to the CO2 space,inspectors noted that the instructions for operation of the installed CO2 system were confusing and not clear for the machinery & cargo spaces. If the box containing controls is to be locked, a key to the box shall be provided(FSS 5/2.2.2.2). It was observed that the key to open the control box for the cargo hold was missing. Maintenance, testing and inspection shall be carried out having due regard to ensuring the reliability of firefighting systems(SOLAS III/14.2.2.1). The armored cabling for CO2 discharge solenoids was found to be severely wasted and improperly installed.

General arrangement plans shall be permanently exhibited for the guidance of the shops officers showing the location of the various fire-extinguishing particulars of the ship. Fire control and safety plan is out of date in identifying the equipment located in the port side fire control locker and other other spaces on the ship. Further, it was noted that safety equipment identified on the class approved plan was out of date. Fire Control and safety plan was last verified and stamped by class at vessel new construction approximately 15 years ago.

In addition to the requirements contained elsewhere in the present regulations, ships shall be maintained in compliance with the electrical requirements of a classification society or with applicable standards of the Administration. Multiple cable ways and cable hangers throughout the vessel(on deck) were identified to be wasted, holed, and/or no longer attached as designed. In addition to this, armored cables for remote winch controls leading from the forecastle deck were found to be severely corroded and the cable hangers wasted.

All ships of 3,000 gross tonnage and upwards shall have: a 3 GHz radar, or where considered appropriate by the administration, a second 9 GHz radar. Vessel's secondary marine radar is inoperable.

During inspection of the rescue boat it was found that the starting battery had a bad terminal connection. Upon further inspection of the rescue boat inspectors discovered a bad clutch linkage resulting in the rescue boat unable to operate with astern propulsion.

Most recent fire extinguisher servicing report showed all fire extinguishers were good with the exception of nine extinguishers. Spot Checked seven of those extinguishers and they still had stickers that read “do not use CO2 cartridge due for replacement” or other very similar verbiage. This led to an expanded inspection and found another 17 extinguishers with the same stickers that were not listed on the service report.

Any other machinery space shall be adequately ventilated appropriate for the purpose of that machinery. The vessels exhaust air fan in the steering gear room is non-operational.

Means shall be provided whereby normal operation of propulsion machinery can be sustained or restored even though one of the essential auxiliaries becomes inoperative. Special consideration shall be given to the malfunction of an air compressor and receiver for starting or controlling purposes. Vessels number 3 start air compressor was inoperable.

During inspection of the rescue boat inspectors discovered a bad clutch linkage resulting in the rescue boat unable to operate with astern propulsion. The following restrictions apply during transit from Houston, TX to Norfolk, VA. These are in addition to items identified in Vessel's risk assessment: 1) No planned maintenance shall be conducted on the main deck or personnel working aloft over the water at any deck height. 2)There shall be 2 person accountability if personnel are on the main deck. 3)A system to account for personnel that must be on the main deck to include coming and going

An inspection for certification is a prerequisite of the reissuance of a certificate of inspection. Vessel was not ready for inspection including several required inspection items such as bridge safety and navigation and lifesaving. Provide attestation of completed class survey upon completion.

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Due to work being done on the vessel, MIs were unable to conduct drills to evaluate crew competency during emergency situations.

Port/Stbd life jacket bins located at life boat embarkation points are not permanently mounted, instead, they are tied with rope to the stanchions. Permanently mount the port and stbd life jacket bins to the satisfaction of Class. SOLAS 2014 (Cons) III/20.2

Fire fighting systems shall be kept in good working order. The fire main hydrant valves on main deck are excessively corroded - some may need replacing. Clean/replace to the satisfaction of Class. SOLAS 2014 (Cons) II-2/14

The arrangements for the utilization of oil used in pressure lubrication systems shall be such as to ensure the safety of the ship and persons on board. The main engine high pressure servo oil casing is cracked and leaking oil. The crack has been siliconed, but appears to be failing. Repair to the satisfaction of Class. SOLAS 2009 (Cons) II-2.4.2.3.1

Piping systems shall be so installed as to reduce to a minimum any danger to persons on board , due regard being paid to hazards. Crew has wrapped the evap's raw water discharge elbow with a soft patch. Repair to the satisfaction of Class. SOLAS 2004 (Cons) II-1.26.1

Ventilator openings shall be provided with weathertight closing appliances. The galley mushroom ventilator located on the stbd side near the life boat is wasted and not weathertight. Repair to the satisfaction of Class. ICLL as modified '88/'03 Annex I/19

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations. The iridium phone satellite cable goose neck on the stbd side on top of the wheel house has wasted in half and has been questionably fiberglassed. Repair to the satisfaction of Class. SOLAS 2014 (Cons) I/11

Machinery shall be so installed as to reduce to a minimum any danger to persons on board , due regard being paid to hazards. The Suez Davits rotational gear guard has come loose exposing the gear. Repair to the satisfaction of Class. SOLAS 2004 (Cons) II-1.26.1

The main engine has excessive oil accumulated around cylinders. Crew cleaned to the satisfaction of CG. 74SOLAS(04)II-2/4.1.1

On main deck #1 Engine room supply vent has hanging wires, ground wire disconnected and a control box just hanging, properly hang so not to cause injury when handled or touched in the normal manner.

Aft louvers for the emergency generator will not close on its own. Louvers need to be pushed close by hand. Systems shall be kept in good order so as to ensure their required performance if a fire occurs for the protection of openings.

Fire door latch leading out of hospital is frozen in place and will not latch. Systems shall be kept in good order so as to ensure their required performance if a fire occurs.

Wiring shall be installed and supported in such a manner as to avoid chaffing or other damage. The wiring leading to the CO2 control box and CO2 alarm relay box are exposed and not protected by the armor cabling.

Fire fight appliances shall be kept in good working order and ready for immediate use. The fire dampener on aft mooring deck for CO2 space is wasted and not operational.

Vessel is to have one tug of 3,000 horsepower and one tug of 2,000 horsepower, or one tug of 5,000 horsepower standby the vessel until the vessel's unsafe conditions have been rectified to the satisfaction of the Coast Guard Marine Inspector. 33 CFR 160.111

The hydraulic means of starting EDG is inoperable. Repair and op-test hydraulic starter to USCG satisfaction. Prior to departing next port, install secondary battery bank to supplement primary start batteries. Supplemental batteries shall be of equivalent capacity to the primary bank. An op-test of the secondary battery bank is required to be reported to the USCG.

Prove proper operation of #2 generator to satisfaction of attending class surveyor.

Activation of any detector or manually operated call point shall indicate a visual and audible fire signal. If the signals have not been received within 2 minutes, an audible alarm shall be automatically sounded throughout vessel. While testing fire detection system, MI found no alarm sounded after 2 min delay. Repair and prove function to attending MI or class surveyor.

Cargo ships shall carry at least one rescue boat complying with part 5.1 of the Code. While testing the rescue boat, the vessel crew could not get the throttle to engage. Repair and prove function to attending MI or class surveyor.

Based on the following deficiencies and the overall condition of the MAERSK ATLANTA, an internal ISM audit of the vessel's safety management system is required on next voyage. In addition, an external ISM audit is required by 05NOV16. SOLAS 14 I/19 and SOLAS 14 IX/6.1

Totally enclosed lifeboats shall be provided with hatches which can be closed to make the lifeboat watertight. Upon inspection, MI found one wedge for dogging the hatch closed on the port life boat missing, and one wedge on the STBD lifeboat broken. Replace wedge to make hatch watertight no later than 31 AUG 16. SOLAS 14 III/31.1.1 LSA CODE 4.6.2.2

Lifeboats shall have handrails providing a secure handhold to aid embarkation and disembarkation. During inspection, MI found grab rail on STBD lifeboat broken off/missing. Replace handrail as lifeboat was designed no later than 31 AUG 16. SOLAS 14 III/31.1 LSA CODE 4.6.2.9

To prevent the ignition of combustible materials or flammable liquids, means shall be provided to control leaks and limit the accumulation of flammable vapours. During the inspection, MI found excessive oil in the bilge under Main Engine with visible leaks coming from the accesses to the scavenger air box. Clean oil from bilge to reduce fire risk and make repairs to ME by 7SEP16 SOLAS 04II-II/4

Fire systems shall be maintained and ready for use. During the inspection, MI found the most forward starboard fire main outlet was leaking a steady stream of water while under pressure. Repair valve to prevent leaking no later than 7 SEP 2016. SOLAS 04 II-2/14.1

Cables and wiring shall be installed and supported in such a manner as to avoid chafing or other damage. MI found wiring in horizontal runs supported with dry-rotted and broken zip ties. These wires need to be properly supported or removed if no longer necessary. MI also found a wire in the BT room laying on a tank with metal shielding corroded off. Replace wire. SOLAS 04 II-1/45.5.5

Machinery and associated piping and fittings shall be of design and construction adequate for the service for which they are intended. During the inspection, the MI found a water leak on the Hi-temperature side of the #2 Diesel Generator coming from a joint/flange with an improper gasket. Replace gasket and stop leak no later than 07 SEP 2016. SOLAS 04 II-1/26

Life saving appliances shall be fitted with retro-reflective material IAW Resolution A.658(16). During the inspection, MI found the STBD lifeboat was missing required tape in some areas, and other tape significantly degraded and needing replacement. Replace tape where necessary no later than 07 SEP 2016 SOLAS 14 III/31.1.1 LSA CODE 1.2.2.7

Ships shall be maintained as originally designed and constructed. During the inspection, the MI found the fan missing from the powered ventilation in the Bosun's store on the bow. Replace fan as system was originally designed. SOLAS 04 II-1/1

Vessel reported tripping of #2 generator due to high voltage, condition of class is issued. Submit class report attesting proper repair and operation of the affected system.

Voyage data recorder not functioning as intended, in that while the system is recording to the main unit, it is not recording to the exterior capsule. The Administration shall be satisfied that adequate arrangements are in place to ensure that the performance of the equipment required by this chapter is maintained (46CFR90.05-10/SOLAS 2009 Cons Ed, Chap. V Reg 16.1).
