A ship of 300 gross tons or more constructed after June 30, 1974 must have a fixed container or enclosed deck area under or around each fuel oil or bulk lubricating oil tank vent, overflow, and fill pipe. Pollution for containment on port side main deck for boiler tank clarified oil holed. 74 SOLAS(04) II-1/40.1.3

The company and the ship shall comply with the requirements of the International Safety Management Code. The company should establish procedures, plans, and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship ans protection of the the environment. Vessel was found to have expired medication in the hospital space cabinet and fridge. Vessels crew has no clear instructions on how to properly maintain inventory logs and when to dispose and replace medication. Company shall establish procedures for maintaining medical inventory and implement these in the vessels SMS. 74SOLAS(2020) IX/3.1 ISM Code Part A/7

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company, in meeting these requirements, the company should ensure that: records of these activities are maintained. Multiple Lockout-TagOut tags were identified during the course of the inspection that were either completely void of the information or missing significant portions of information required by Maersk Line, Limited (MLL) Lockout-Tagout procedure. Records reviewed during the inspection showed that the [name withheld] had never completed an Audit of the Lockout-Tagout log as per MLL requirements. During the inspection, the MLL procedure presented to the inspectors, Document ID 07-03-04-900 dated 22 September 2017 was found to be out of date as per the procedure in DNV Ship Manager. Vessel is to conduct an audit of Lockout-Tagout procedures and current tags in place. In addition, training shall be held with applicable crewmembers of the proper Lockout- Tagout procedures to be followed. 74 SOLAS (20) IX/3.1 ISM Code 10.2.4

In a ship in which oil fuel is used, the arrangements for storage, distribution and utilization shall ensure safety of ship and persons onboard. Identified the following leaks: Main engine at fuel pumps, oil leaks at crank case doors on all cylinders, lube oil purifier for #3 seal, #2 generator lube oil leak at outlet gasket, #1 generator at piping aft of filter housing.

The following fire protection systems shall be kept in good order so as to ensure their required performace if a fire occurs: Fire detection and fire alarm systems. Flame detector in purifier room rendered inoperable by ship's staff while testing. Vessel engineer used heat gun instead of required light as per approved testing procedures.

Fire doors in main vertical zones and stairway enclosures shall be self-closing. Fire door from stairway to F Deck and door leaking to ECR failed to self closed.

Alarms shall be maintained until accepted and the visual indications of individual alarms shall remain until the fault has been corrected, when the system shall automatically reset to the normal operating condition. 2 active alarms onboard that crew is unable to identify and resolve; "WHR Boiler Operation Abnormal" and "Overload Sludge Burning Pump".

The company should establish procedures to ensure the ship is maintained in conformity with the provisions of the relevant rules/regulations. Based on the overall material condition of the vessel as substantiated by the objective evidence cited above, and 117 overdue maintenance items related to machinery. An additional external verification is required.

Before the ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. Starboard lifeboat culd not start. Rectified by permanent repair of new fuel intake hose. Rectified on the spot.

When in use, the lower end of the accomodation ladder, as well as the pilot ladder, shall rest firmly against the ship's side. Vessel did not implemented procedures and equipment (magnets and rigging) designed for securing combination ladder to the ship's side prior to taking pilots for its inbound transit to Charleston.

All electrical apparatus shall be constructed and so installed as not to cause injury when handled or touched in the normal manner. In crew laundry room there is a washer and dryer incorrectly installed.

All lifesaving appliances shall be in working order and ready for immediate use. The lines on the lifebuoys that connect the self igniting lights are deteriorated on a majority of the lifebuoys.

Maintenance shall be carried out to ensure reliability of lifesaving appliances. Embarkation ladders on majority of launching stations are deteriorated.

The #5 Injector Control Unit (ICU) has been found faulty on the main propulsion engine. Upon completion of the repairs to the #5 ICU, and after operational tests have been completed, forward the technician’s report to this office for review prior to the due date.

It was reported that the vessel's S-Band radar failed while in port due to a malfunctioning power unit. MSU Savannah issued an LOD to depart the Port of Savannah. The vessel will make a coastal voyage to Norfolk where a new power unit will be deilivered and replaced.

3 cm Radar is out of commission. Prove proper operation.

Redundant vital auxiliaries must automatically transfer to the backup units upon failure of operating units. Vessel's main engine secondary jacket water pump (stbd) is currently inoperable (motor bearings). Deficiency must be addressed to the satisfaction of CG marine inspector or vessel's classification society.

One of the Gyro Compass Repeater failed.
