Vessels crew has no clear instructions on how to properly maintain inventory logs and when to dispose and replace medication. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company, in meeting these requirements, the company should ensure that: records of these activities are maintained. Multiple Lockout-TagOut tags were identified during the course of the inspection that were either completely void of the information or missing significant portions of information required by Maersk Line, Limited (MLL) Lockout-Tagout procedure. Records reviewed during the inspection showed that the [name withheld] had never completed an Audit of the Lockout-Tagout log as per MLL requirements. During the inspection, the MLL procedure presented to the inspectors, Document ID 07-03-04-900 dated 22 September 2017 was found to be out of date as per the procedure in DNV Ship Manager. Vessel is to conduct an audit of Lockout-Tagout procedures and current tags in place. In addition, training shall be held with applicable crewmembers of the proper Lockout- Tagout procedures to be followed. 74 SOLAS (20) IX/3.1 ISM Code 10.2.4
