The life buoy grab line shall be 3/8"-inch diameter polyethylene, polypropylene, or other suitable buoyant type synthetic material having a minimum breaking strength of 1,350 pounds. The grab line on all eight life buoys were either severed or deteriorated. Replace all life buoy grab lines to original specification.

For transfer operations between sunset and sunrise, deck lighting that adequately illuminates transfer operations is required. Deck lighting was inop around the transfer area. Do not conduct transfer operations between sunset and sunrise until cleared.

The last 1.0 meter of vapor piping upstream of the vessel vapor connection and each end of a vapor hose must be painted in sequence of red/yellow/red. The width of the red bands must be 0.1 Meter and the width of the yellow band must be 0.8 Meter and labeled with the word "VAPOR". The color scheme of the vapor line did not meet requirements.
