The crew, with Captain's knowledge, are using trash bag with duct taped tube to capture cargo vapor at desk tank sampling point. Cargo vapor is then used to test fixed gas detection sampling points in motor room, compressor room and void spaces. Captain acknowledged this is not an approved procedure. Prior to cargo operations.

Testing should be carried out at regular intervals. Suitable equipment and span gas for this purpose should be carried on board. Span gas bottle are too small to test fixed gas detection sampling points in motor room, compressor room and void spaces.

The container shall be marked with date last serviced. Starboard side life raft did not have an updated marking with recent service date.

The deck water spray system should be capable of covering all areas mentioned in 11.3.1 with uniformly distributed water-spray of at least 10 l/m2 per min... Vessel had numerous clogged sprinkler heads on cargo tank domes #1 and #3 causing ineffective coverage. This deficiency was satisfactorily rectified on board in presence of PSCO's. - IGC Code 93/ 11.3.2

If cargo control room is designed to be a gas-safe space, instrumentation should... be designed to prevent any escape of gas into the atmosphere of that space. Gas detection cabinet had a fan installed. - IGC Code 3.4.2

Cylinders must always be secured and when not in use, must be stowed in a rack. 03 oxygen and 03 acetylene cylinders were found stowed on the deck, not in a rack. - 46 CFR 147.60

07126-Precautions shall be taken to prevent any oil that may escape under pressure from any pump, filter or heater from coming into contact with heated surfaces. PSCO found oil leaks and excessive pooling of oil on and around the main engine #6 cylinder.

14199-The condition of the ship ands its equipment shall be maintained to conform with the provisions of the present convention to ensure that the ship in all respects will remain fit to proceed to sea without presenting and unreasonable threat of harm to the marine environment. PSCO observed a patch on the water supply line on the gravity tank for the Oily Water Separator. The patch was removed by the crew, revealing a pin hole leak in the line.

The company should ensure that the appropriate corrective action is taken for any reported non-conformity. The ship reported a malfunctioning relay module of the burner controller on the Inert Gas Generator on February 25th 2020. The relay module was not ordered until July 16th 2020. The hold spaces have not been inerted as required in the ICG code 9.2.1 since January 7th 2020. There were no additional mitigating safety measures implemented for the ship to follow and maintain an inert atmosphere in hold spaces. Also, the crew was unable to provide documentation attesting to the proper notification to the vessel's classification society in regards to the malfunctioning relay module of the burner controller on the Inert Gas Generator. Deficiencies #2 through #4 make up a systematic breakdown of the vessel's Safety Management System. An external audit is required. 74 SOLAS (14) IX/3.1 ISM 10.2.3

The condition of the ship and it's equipment should be maintained to conform with the provisions of the Code to ensure that the ship will remain fit to proceed to sea without danger to the ship or persons on board. The Inert Gas Generator oxygen content meter cannot operate properly without bypassing the flow regulator for the sample gas tubing. Subsequently the hold spaces have not been inerted IAW IGC Code 9.2.1 since 07JAN2020 as per their Dry Inert Gas Generator Operations Log. 74 SOLAS (14) VII/13.1 93 IGC 1.5.3.1

Electrical equipment, cables and wiring should not be installed in hazardous locations unless it conforms with the standards. Bonding cabling for lighting near Cargo Tank #3 was missing. 74 SOLAS (14) VII/13.1 93 IGC 10.1.4

The gas detection equipment should be capable of sampling and analyzing for each sampling head location sequentially at intervals not exceeding 30 minute. The fixed gas detection sampling heads located at the top and bottom of the cargo compressor room were equipped with valves which would allow for isolation of the sampling heads. Due to the arrangement the sampling heads did not allow for each sampling head location to be sequentially analyzed at intervals not exceeding 30 minutes per the IGC code. 74 SOLAS (14) VII/13.1 93 IGC 13.6.8

01305-log-books/compulsory entries- changes in set pressures should be recorded in the ship log and a sign posted in the cargo control room, if provided, and at each relief valve, stating the set pressure.
