The integrity of a stairwell shall br maintained by "A" class doors at every level. The doors shall be of self closing type.
Each self closing door in the stairtower at the upper deck, poop deck, boat deck, and captain's office deck would not fully self close. Crew adjusted all door's closing devices to fully close to meet regulation.

If a vessel's voyage is less than 96 hours, then the vessel must submit an NOA before departure but at least 24 hours before arriving at the port or place of destination. While vessel was en route from Meldrum Bay, Canada to Bay City, MI, a voyage of less than 96 hours, the vessel submitted their ENOA to the National Vessel Movement Center on September 17, 2024 @ 14:55 GMT (10:55 EST) with an ETA of September 17, 2024 @ 16:00 EST. Additionally, on September 17, 2024, when Port State Control Examiners were able to screen the vessel's NOA, the vessel was seen to be already in U.S. Waters in process of docking in Bay City, MI. Vessel arrived in Bay City, MI with only 6 hours notice.

Resolution: Deficiency was rectified on the spot.
The routine maintenance of the oily water separating equipment and the bilge alarm must be clearly defined by the manufacturer in the associated operating and maintenance manuals. All routine and repair maintenance must be recorded.
PSC Examiners found the piping and valves bewteen the oily water separator and overboard discharge to be disassembled, preventing the system from being tested or operating as designed.
