The station bill of a vessel of more than 19.8 meters (65 feet) in length required to have more than four crew members, must set forth the special duties and duty station of each crew member for various emergencies. The duties must include at least the following and any other duties necessary for the proper handling of a particular emergency.(1) The closing of hatches, airports, watertight doors, vents, scuppers, and valves for intake and discharge lines that penetrate the hull, the stopping of fans and ventilating systems, and the operating of all safety equipment;(2) The preparing and launching of survival craft and rescue boats;(3) The extinguishing of fire; and(4) The mustering of passengers including the following:(i) Warning the passengers;(ii) Assembling the passengers and directing them to their appointed stations; and(iii) Keeping order in the passageways and stairways and generally controlling the movement of the passengers. The station bill must be posted at the operating station and a conspicuous location in each crew accommodation space. Create and post an appropriate station bill for the vessel crew.

The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Emergency instructions indicate use of life line and instruct crew to enter water if necessary to retrieve passengers. Master advised he would not instruct any crewmember to enter water. Life lines are not provided to crew. Revise procedure to align with Master's/company expectations and available equipment.

Each member of the crew shall participate in at least one abandon ship drill each month. If more than 25% of the crew have not participated in an abandon ship drill in the previous month a drill must be conducted before the vessel gets underway with passengers aboard. Abandon ship and man overboard drills and training shall be logged for review by the USCG including the date and general description of the drill/training. Drills were not carried out as required or logged.

A fire drill must be conducted at least once each month and shall be logged or documented for review by the USCG including date and general description of the drill and training topics. Drills were not carried out or logged.

Emergency instructions will be mounted at the operating station or carried aboard and available containing the actions to be taken in the event of fire, flooding, heavy weather or man overboard conditions. Emergency instructions were not verified.

Each officer employed upon any vessel subject to provisions of this subchapter must have his or her license or MMC on board and available for examination at all times when the vessel is operating. Documents were not on board at time of inspection.

Employee Assistance Program (EAP): each EAP must include at least the following: display and distribution of a community service hotline telephone number for crewmember assistance and display and distribution of the employer's policy regarding drug and alcohol use in the work place. No postings of the policy or phone number were found.

On a vessel which the number of passengers permitted on the upper deck is limited by stability criteria per the stability letter, the max number of passengers allowed on an upper deck must be indicated by a durable marking of at least 1 inch numbers and letters at the entranceway to that deck. Upper decks were not labeled at the entranceway with max passengers allowed.

Each vessel must be equipped with at least to operable portable battery lights, one at the operating station and one at access to propulsion machinery. Operating station light was found without batteries.

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship, man overboard and fire drill be held under simulated emergency condition specified by the inspector. Drills are required to be carried out after new crew is hired on board for the summer season. Notify Coast Guard of the date drills will be carried out for attendance by Marine Inspector.

There must be no protrusions in means of escape that could cause injury, ensnare clothing, or damage life jackets, and each door, hatch, or scuttle used as means of escape must be capable of being opened by one person, in both light and dark conditions. The method of opening a means of escape must be obvious, rapid, and of adequate strength. Fasteners used to secure insulation are not covered, and escape hatch from engine room is clearly marked in a contrasting color.

A fire control plan must be posted on the vessel in a location that is accessible and visible to all passengers. The plan must show escape routes, areas of refuge, embarkation stations, the location of fire protection/emergency equipment, compartment titles and hazard classification of accommodation and service spaces, and structural fire protection. Current fire control plan does not include escape routes, areas of refuge and is faded.

All audible and visual alarms must be audible and visible throughout the spaces they are intended to alert. Multiple strobes not working.

Vessel must carry a first-aid kit approved under approval series 160.041. -1st Aid Kit found missing aspirin, eye wash, inhalants

:VHF radio DSC/Distress button has no MMSI number registered to it. Vessel shall input MMSI number, and also ensure VHF radio is linked to GPS.

Light list, coast pilot, and charts are outdated. Vessel shall update or renew all three.

Vessel shall conduct fire, and MOB drill to satisfaction of USCG within 30 days.

Hose at aft fire station is holed.

Battery in ringbuoy float light is expired. Replace battery.

Lifeline is required to be 60' in length. The lifebuoy lifeline provided measures less than 60'.

Time delay did not pass testing; activiated too early. Replace componennt to ensure fire suppression system complies with original system plan approval. No personnel permitted in the machinery space while equipment is in operation.
