The owner or operator of all unmanned tank barges shall ensure that one English language copy of the plan section listed in paragraph (c)(9) of this section and the Coast Guard approval letter is maintained aboard the barge. An electronic copy of the VRP is authorized. 1030(i)(2) No copy of VRP approval letter on board, associated tow vessel not alongside.

The term flame screen means a fitted single screen of corrosion-resistant wire of at least 30 by 30 mesh, or two fitted screens, both of corrosion-resistant wire, of at least 20 by 20 mesh, spaced not less than 1/2 inch or more than 11/2 inches apart. 3 of the 4 flame screens for the tank vents were found with holes.

) During cargo transfer operations, inland oil barges must have appropriate equipment and supplies ready for immediate use to control and remove on-deck oil cargo spills of at least one barrel. (b) The equipment and supplies must include— (1) Sorbents; (2) Non-sparking hand scoops, shovels, and buckets; (3) Containers suitable for holding recovered waste; (4) Emulsifiers for deck cleaning; and (5) Protective clothing. The cargo discharge piping of all tank vessels shall be tested at least once each year for tightness, at the maximum working pressure. Manifold not marked

Lifebuoys must have the following attachments and fittings: (i) At least one lifebuoy on each side of the vessel fitted with a buoyant lifeline that is— (A) At least as long as twice the height where it is stowed above the waterline with the vessel in its lightest seagoing condition, or 30 meters (100 feet) in length, whichever is the greater; (B) Non-kinking; (C) Not less than 8 millimeters (5/16 inch) in diameter; (D) Of a breaking strength which is not less than 5 kiloNewtons (1,124 pounds-force); and (E) Is, if synthetic, a dark color or certified by the manufacturer to be resistant to deterioration from ultraviolet light. (ii) At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. The self-igniting light may not be attached to the lifebuoys required by this section to be fitted with lifelines. Life bouy installed was wasted and life line was wasted also.

For vessels having a square bow, the name of the vessel must be marked on some clearly visible exterior part of the bow in a manner to avoid obliteration. Tires were covering vessel name

Approved portable and semi-portable extinguishers must be installed in accordance with table 34.50–10(a) of this section. Fire Extinguisher was not serviced

The operators of each vessel and facility engaged in the transfer operation shall retain a signed copy of the declaration of inspection on board the vessel or at the facility for at least 1 month from the date of signature. No DOI present

A vessel owner or operator required by § 155.1035, § 155.1040, or § 155.5035 to have a response plan shall conduct exercise as necessary to ensure that the plan will function in an emergency. Both announced and unannounced exercises must be included. No proof of exercise was conducted,

A ship, except a ship of less than 26 feet in length, must have a placard of at least 5 by 8 inches, made of durable material fixed in a conspicuous place in each machinery space, or at the bilge and ballast pump control station,

Entries shall be made in the records for tank barges with respect to the following: (1) Inspections and tests of firefighting equipment. Once every year. Vessel fire extinguishers had no record of servicing this year.

No person may transfer oil or hazardous material to or from a vessel unless each person in charge, designated under §§154.710 and 155.700 of this chapter, has filled out and signed the declaration of inspection form described in paragraph (c) of this section. Vessel is not completing DOI when transferring from the barge. Vessel can not transfer cargo with out conducting DOI.

An exercise of the entire response plan, which must be conducted every 3 years. The vessel owner or operator shall design the exercise program so that all components of the response plan are exercised at least once every 3 years. All of the components do not have to be exercised at one time; they may be exercised over the 3-year period through the required exercises or through an area exercise. Provide documentation has been completed.

Each battery clearly marked with an expiration date and used in an item of survival equipment must be replaced during the annual inspection if the expiration date has passed. Float light battery was expired

Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port required to be marked on the stern of the vessel under §67.123 of part 67 of this chapter. Ring buoy not marked

For vessels having a square bow, the name of the vessel must be marked on some clearly visible exterior part of the bow in a manner to avoid obliteration. The name and hailing port must be marked on some clearly visible exterior part of the stern. Above requirements blocked by fendering tires.

Each vessel that transfers oil in a port subject to the jurisdiction of the US must have onboard a valid vessel response plan. Vessel's response plan expired on 26Feb2019.

Each tank vessel operating in salt water must undergo two internal structure examinations within any five year period. Vessel was due for an internal structure exam on 27Feb2019.
