The owner, master, or person-in-charge of a tank vessel must ensure that portable extinguishers are maintained in accordance with NFPA 10. Portable fire extinguishers shall be installed in a bracket incorporating releasing banks or straps or in an approved cabinet or wall recesses. All 03 fire extinguishers mounted on deck were not adequately secured by straps or bands.

Upon inspection, Marine Inspectors discovered the Buffalo Marine DOI form did not include the name of the cargo being transferred on their form, nor was there a designated spot on the form to record this information. Prove to the satisfaction of the Marine Inspector the name of the products being transferred is included on the DOI.
