The central MSW pump 1S developed a blow out hole roughly 3/16" in diameter in the discharge piping.

The purpose of this regulation is to contain a fire in the space of origin. Power supply and attached fuse for the synchroconverter failed and breaker could not be reset leaving only 50% power available to starboard shaft.

Provide proper launching arrangement for the life rafts port aft (3) units and the stbd (2) units. Arrangement shall match the posted procedures.

The vessel’s port fire pump is currently inoperable. A vessel of this size and type is required to be fitted with two independently driven fire pumps. The vessel may not proceed to sea until proper permanent repairs are made to the port fire pump and it is tested in the presence of a class surveyor.
