As per "The Code" 1.2.2 Safety management objectives of the company should provide for safe practices in ship operation and a safe working environment.
As stated in the email attached you said that you "Vessel has taken out of service the pilot ladder in question and utilizing spare pilot ladder that meets IMO Standards..."
As you had the proper pilot ladder on board an choose not to use it questions the safe practices when embarking and disembarking when using a pilot ladder. A Internal Safety Management Audit is required on safe work practices focusing in on use of the pilot ladder.
