A Safety Management Certificate, shall be issued to a ship that verifies that shipboard management operates in accordance with the approved safety-management system. As per "The Code" 1.2.2 Safety management objectives of the company should provide for safe practices in ship operation and a safe working environment. As stated in the email attached you said that you "Vessel has taken out of service the pilot ladder in question and utilizing spare pilot ladder that meets IMO Standards..." As you had the proper pilot ladder on board an choose not to use it questions the safe practices when embarking and disembarking when using a pilot ladder. A Internal Safety Management Audit is required on safe work practices focusing in on use of the pilot ladder. The audit report to be provide to [email withheld] by 09 December 2024

It was reported that the ships pilot ladder was unsafe. As per the pictures provide there are 17 steps between the spreads and the maximum is 9 as per IMO Resolution A.1045(27) Pilot [name withheld] shall be no transfer of people using the pilot ladder until a ladder that meets the IMO Resolution A.1045(27) Pilot [name withheld] is in place.

Every oil tanker of 150 gross tonnage and above and every ship of 400 gross tonnage and above other than an oil tanker shall be provided with an Oil Record Book Part I (Machinery space operations).
