Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vsl suffered temporary loss of steering due to a bad motor starter for the steering system, Repair to factory and to the satisfaction of the TPO and the MI. 46CFR143.205(a)

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protection against hazards to life, property, and the environment. Vessel allided with the Bayou Sorrel locks causing a crew member to fall overboard, member was recovered via skiff with no injuries. Conduct survey to assess any damage, provide report to the CG and provide corrective action report identifying cause of allision and steps to prevent it again.
