Key personnel must complete training in 33CFR101.650(d)(2) by January 12, 2026. Company does not have a CYSO and all key personnel have not yet completed the training as of this inspection. 33 CFR 101.650(d)(4)

A pre-engineered fixed gas fire extinguishing system must: Automatically shut down all power ventilation systems and all engines that draw intake air from within the protection space. Stbd side engine room machinery did not automatically secure when pre-engineered system was tested. Make pre-engineered system function as designed. 46 CFR 118.420(a)(3)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: External inspection of fuel tanks and inspection of tank vents, piping, and pipe fittings. Port center void had strong fuel odor. Locate source of fuel odor and make necessary correction. 46 CFR 115.804(d)

Fire hose must be lined commercial firehose that conforms to UL 19 “Standard for Safety for Lined Fire Hose and Hose Assemblies” or hose that is listed and labeled by an independent laboratory recognized by the Commandant. Fwd hose has pin holes, replace as required. 46 CFR 118.320(b)(3)

On a vessel of more than 65 feet in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. PA system did not work on fantail or upper deck. Make necessary repair.46 CFR 121.610(b)

Life jackets must be stored in convenient places distributed throughout accommodation spaces. On upper deck there is marking for life jackets on aft bench stating that life jackets are stowed under seats when in fact they are stowed in storage bins fore and aft. Make signage accurate and ensure passenger safety brief aligns with signage. 46 CFR 117.78(a)

An adult life jacket must be provided for each person carried on board a vessel. On upper deck, 01 adult life jacket was wasted and 05 adult life jackets had wasted retro-reflective tape. Replace as required. 46 CFR 117.71(a)

Each officer employed upon any vessel subject to the provisions of this subchapter must have his or her license or merchant mariner credential on board and available for examination at all times when the vessel is operating. Mate did not have MMC on board, provide copy of MMC. 46 CFR 122.402

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Port and stbd machinery spaces are both equipped with two ventilation closure pull cables. One on each side did not activate closing mechanism. Diagnose, conduct operational test, and provide use for all ventilation pull cables on port and stbd sides. Make repairs and label as necessary. 46 CFR 119.465(h)

Each vessel that relies on electricity to power the following loads must be arranged so that the loads can be energized from two sources of electricity: The vital systems listed in 119.710 of this chapter. The following vital systems on board the vessel rely on electricity to operate: fire main and steering system. The vessel is equipped with two sources of electricity (port and starboard generator). The port generator is inoperable. Make necessary repairs to port generator, provide technical report, and conduct satisfactory operational test to the satisfaction of the attending marine inspector prior to carrying passengers. 46 CFR 120.310(a)(1)(i)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Submit CG Form 2692. Determine root cause and analysis. Conduct satisfactory sea trials prior to operating. 46 CFR 182.100

Fire pump pressure gauge was found to be inoperable. A spare was onboard and replaced in the presence of the marine inspector.

Before getting underway on a voyage or as soon as practicable thereafter, the MASTER of the vessel shall ensure that suitable public announcements are made informing all passengers of the location of emergency exits, survival craft embarkation areas, ring life buoys, stowage location of life jackets. USCG MI observed on 25MAR2023 that the passenger orientation was inoperable in the enclosed passenger cabin and notified [name withheld] Then again on 3MAR32 the same CG MI was on the vessel and noted that the passenger orientation was not broadcast in the upper deck. Make repairs prior to 9MAR2023. In the mean time you may utilize one of two options to remain operational. Option 1. Close the upper passenger deck to passengers. Option 2. Conduct Safety Orientation that includes all required information provided verbally and visual by a crew member demonstration each voyage with passengers. 46 CFR 122.506

Construction and arrangement of the vessel must allow the safe operation of the vessel. Attending MI noted that there is window missing in the main passenger cabin that is covered in PVC backer-board. Repair/Replace 46 CFR 116.100

Main deck forward weathertight door has a 4" hole at the bottom adjacent to the deck coaming. Provide repair plan proposal to [email withheld] within 30 days.

A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protection space. Upon crew testing of pre-engineered fixed fire extinguishing system (fireboy), marine inspectors observed the system failed to shutdown all machinery within the machinery spaces and the override systems were not operational. Prove proper operation of fireboy shutdowns and overrides for port and starboard machinery spaces in the presence of USCG marine inspectors.

Safety Drills were not logged. Last entries were in 2019.

Senior Deckhand not designated in writing per NVIC 1-91. The employment and training of qualified deckhands is the responsibility of the marine employer (46 CFR 15.103).

Placed 17 Adult and 1 child pfd out of service. Passenger count reduced to 183 until new pfds are provided.

Starboard generator water pump inoperable due to broken water pump. Questioned crew who stated the water pump broke two weeks prior to the inspection. New water pump on order. Vessel is equipped with two power sources including port generator, main propulsion battery chargers, and batteries IAW 46 CFR 120.310.

All (04) fireboy bottles were secured with safety pins. Questioned crew and they were unaware the safety pins were in place. Observed crew remove all safety pins and discussed the critical importance of examining fire equipment before operating.

Forward remote fuel shutoff valves not marked IAW 46 CFR 122.608

Vessel experienced choppy movement of port rudder during testing of system in synchronized mode. Upon rebooting of system, synchronized operation was inoperable. Vessel normal operation is in synchronized mode. Vessel prohibited from operation until diagnosed and corrected to satisfaction of attending USCG Marine Inspector. Vessel is allowed to transit locally between St Thomas and St John to effect repairs and conduct trials.

Installed Fireboy Pre-Engineered Fixed Firefighting Systems did not shut down all internal combustion machinery in effected spaces during test. Demonstrate shutdowns upon activation of system.

Port steering bilge high level alarm visual indicator not operable.

Remove excess oil in both engine room bilges and on inboard side of port generator.

All inflatable buoyant apparatuses found with painter line disconnected from weak link.

Vessel suffered a loss of propulsion on the port engine. Diagnose cause. Repair and demonstrate proper operation to attending U.S. Coast Guard Marine Inspector prior to vessel movement.

Steering system hunting when in synchronized mode. Demonstrate proper operation.

Starboard rudder angle indicator inoperable. Demonstrate proper operation.

Vessel reported suffering a steering loss at approximately 0630 on 10JAN2019 due to loss of hydraulic fluid. Prior to carrying passengers, demonstrate proper operation of steering system.

Port side fixed firefighting system found depleted. Provide causal analysis and repair/corrective action proposal and complete repair/corrective action.

Rectify fixed fire detection fault.

Provide servicing certificate for fixed gas firefighting system.

Diagnose and repair port rudder hunting.

Provide current Certificate of Documentation

Repair leaking air conditioning piping in center void.

Repair leaking fire pump.

Provide training and designation of Senior Deckhands prior to utilizing Senior Deckhand in lieu of mate.

Demonstrate proper operation of fire detection system prior to carrying passengers.

Complete fire drill, abandon ship drill and man overboard drills prior to carrying passengers.

Show proper operation of main engines, gauges, and steering.

Forepeak tanks gravity drain to vent compartments through collission bulkheads. Provide repair proposal to USCG Marine Inspector.

Provide MMSI and GPS input to DSC radio.

Service IBAs.

Pre-engineered fixed firefighting systems installed in both engine rooms were found with discharged cylinders. Prior to carrying passengers, install functioning and serviced fixed firefighting systems in both engine rooms to the satisfaction of the attending USCG marine inspector.

Service all expired portable fire extinguishers onboard the vessel.

Service fixed fire extinguishing system in port and starboard engine rooms.

Prove operation of port and starboard bilge pumps. Pump was not able to take suction and dewater.

Prove operation of emergency lighting in port and starboard mid-compartment and port and starboard steering gear compartment.

Replace missing covers on lighting fixtures in port and starboard steering compartments.

Prove operation of service lighting in port side steering compartment.

Replace broken emergency lighting fixtures in port and starboard steering compartments.

Prove operation of pressure gauge servicing fire pump.

Make name of vessel readable on all life ring buoys.

Program radio with MMSI number for DSC and prove proper operation.

Provide current tide tables onboard for operating area.

Replace one missing red distress flare.

Provide monthly EPIRB testing log.

Provide an approved first aid kit onboard vessel.

Make vessel available to conduct drills.

The owner failed to make vessel available to have an annual inspection within 3 months after COI anniversary of 28OCT2013. Inspection must have been conducted by 28JAN16.
