Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Repairs or alterations that affect the safety of the vessel include, but are not limited to: damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems. The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. FAST CAT suffered a port main engine starting battery fire causing damaging the batteries, electrical wiring and the port generator. Determine the cause of the fire, complete all necessary repairs to original plans, provide a licensed marine electrician report addressing the scope of work completed and provide any associated invoices/ receipts. Conduct satisfactory sea trials in the presence of USCG Marine Inspector after repairs are completed. 46 CFR 176.700

The following spaces must be equipped with a fixed gas fire extinguishing system, in compliance with § 181.410, or other fixed fire extinguishing system specifically approved by the Commandant, except as otherwise allowed by paragraph (b) of this section: (1) A space containing propulsion machinery; (2) A space containing an internal combustion engine of more than 37.3 kW (50 hp);… FAST CAT suffered a port main engine starting battery fire, causing the port side engine room fixed fire extinguishing system (Fireboy bottle) to discharge. Ensure Fireboy is repaired/reinstalled IAW manufacturer recommendations. Conduct satisfactory inspection of Fireboy system in the presence of USCG Marine Inspector after repairs/reinstallation is completed. 46 CFR 181.400(a)

The design, manufacture, installation, and operation of fire detection and alarm systems must be in accordance with either: (1) Sections 76.27-5 through 76.27-35; or (2) SOLAS Chapter II-2, Regulation 7 and FSS Code Chapter 9 (both incorporated by reference, see § 76.01-2) as detailed in § 76.27-70. Vessel experienced two battery fires in less than two years. Additionally, due to the vessel’s construction of both wood and composite materials, the risk of rapid-fire spread is greater than a vessel not constructed of combustible materials. As a result, vessel install detectors, in the form of both heat and smoke, above both the port and starboard generators within each engine room. The heat and smoke detectors must be able to be monitored remotely from the wheelhouse and the heat detectors must trigger at no higher than 135 degrees. The fire detection and alarm system must be of an approved type and installed in accordance with 46 CFR 76.27. IAW 46 CFR 176.800(a), MSU STT utilized OCMI authority & good marine practice to require fire detection and alarm system. 46 CFR 76.27 & 46 CFR 176.800(a)

Except as provided in paragraph (c) of this section and § 177.210 of this part, the owner of a vessel requesting initial inspection for certification shall, prior to the start of construction unless otherwise allowed by the cognizant Officer in Charge, Marine Inspection (OCMI), submit for approval to the cognizant OCMI, at least two copies of the following plans: Electrical installation including, but not limited to: (i) Elementary one-line diagram of the power system; (ii) Cable lists; (iii) Bills of materials; (iv) Type and size of generators and prime movers; (v) Type and size of generator cables, bus-tie cables, feeders, and branch circuit cables; (vi) Power, lighting, and interior communication panelboards with number of circuits and rating of energy consuming devices; (vii) Type of capacity of storage batteries; (viii) Rating of circuit breakers and switches, interrupting capacity of circuit breakers, and rating and setting of overcurrent devices; and (ix) Electrical plant load analysis. Vessel experienced two battery fires in less than two years. As part of marine casualty investigation, unit determined the approved plans do not match the current electrical arrangement on the FAST CAT. I am concerned that electrical alterations have been made without Coast Guard MSC and/or OCMI approval MSU STT utiliized OCMI authority & good marine practice to require full electrical review through MSC as outlined above. 46 CFR 177.202 & 46 CFR 176.800(a)

Each vessel that relies on electricity to power the following loads must be arranged so that the vital systems listed in §182.710l can be energized from two sources of electricity. The starboard generator ECU was damaged due to a short circuit on a chaffed wire. This casualty was not reported to the CG on the date of the occurrence of 13JUL21. Repair stbd generator prior to carrying passengers to the satisfaction of the CG.

The Certificate of Inspection must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations. Properly register new installed EPIRB.

Provide proof of current DAPI enrollment for all crew.

A passenger accommodation space must be maintained to minimize fire and safety hazards. Remove portable gasoline generator from the passenger accommodation space and off the vessel.

Fire extinguishing equipment installed on a vessel in excess of the requirements of §§181.400 and 181.500 must be designed, constructed, installed, and maintained in accordance with a recognized industry standard. Fire alarm and smoke detection system onboard the vessel is not serviced and it is disconnected. Conduct proper system servicing.

Submersible bilge pump discharge lines do not have positive shutoff valves installed at the hull penetrations.

Starboard main engine wet exhaust is leaking water at the engine connection.

Vessel experienced damage including de-lamination of fiber glass and separation of laminated frames in way of the forward third of the center hull. Provide complete damage assessment to be followed by repair proposal by qualified naval architect or marine professional engineer to attending U.S. Coast Guard Marine Inspector.

Demonstrate capability of activating fixed gas fire extinguishing system from outside of the protected space. 46CFR181.420(a)(2)

Provide and demonstrate proper operation of a class A Automatic Identification System (AIS). 33CFR164.46

Provide Safety Radiotelephony Certificate and ship to ship safety radio telephony certificate. 47CFR80.903

Remove spring clip battery cables. 46CFR183.350(d)

Provide suitable hull strainers for main engine raw water intake cooling lines. 46CFR182.420(a)(2)

Complete COI Inspection upon arrival to arrival to new OCMI prior to carrying passengers.

COD shall reflect new owner/operator prior to new zone inspection.
