Nonconductive mats or grating must be provided on the deck in front of each switchboard. The switchboard in the Emergency generator space had no nonconductive matting in front of the emergency switchboard.

Before any vessel leaves protected waters, the master shall ensure that the vessel's exposed cargo hatches and other openings in the hull are closed; made properly watertight by the use of tarpaulins, gaskets, or similar devices; and properly secured for sea. Emergency escape hatch from engine room to main deck was missing a plug for the potential connection of shore power.

Electrical equipment on a vessel must be designed, installed, and maintained to protect crew members, offshore workers, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock, minimize accidental personal contact with energized parts, and prevent electrical ignition of flammable vapors. -Cable connection into the transformer in the emergency generator room is broken. -On the starboard side of the main deck, a wire was loose with no connection.

Electrical equipment exposed to the weather must be watertight. Cable penetration above the pilothouse was not watertight.

Each vessel must be operated in compliance with Section 311 of the Federal Water Pollution Control Act, as amended (33 U.S.C. 1321); and(b) 33 CFR parts 151, 155, and 156. Containment must have mechanical means of closing each drain. Multiple cofferdams were missing mechanical means of closing.

Containers, brackets, racks, and other similar stowage locations for life-saving equipment shall be marked with symbols in accordance with the recommendations of the Organization, indicating the devices stowed in that location for that purpose. -P&S lifebuoy symbols missing -port & stbd embarkation ladder symbols faded/missing SOLAS 20 CHAP 3 REG 20.10

Each generator arranged for parallel operations must have reverse power or reverse current trips. Neither generator tripped during reverse power relay test.

Cargo tanks shall be fitted with visual and audible high-level alarms. A portion of the protective piping that encloses the high-level alarm sending cable within a hazardous zone is disconnected from both ends on the #02 Starboard tank. The cargo approved for back load operations under CG ENG 2025-09-17T055407 requires high level alarms. Repair enclosure prior to conducting back load operations to the # 02 starboard tank. IBC Code 15.19.6

Cargo tanks shall be fitted with gauging devices. Restricted gauging devices not in use shall be completely closed. The restricted gauging device on the #03 port cargo tank is missing the valve handle for open/closure. The cargo approved for backload operations under CG ENG 2025-09-17T055407, dated 26SEP2025 requires restricted gauging. Renew the handle prior to conducting backload operations to the # 03 port cargo tank. IBC Code 13.1.1

No person shall conduct an oil or hazardous material transfer unless a drain is closed by mechanical means. Small discharge containments around hose connection/vents on both port and starboard sides were missing mechanical means for closure. Prior to transfer. 33 CFR 156.120

Vessel shall be kept in a safe and seaworthy condition. Numerous deficiencies were found in conjunction w/ ABS wrote. Vessel shall complete ABS deficiency list prior to operations. 701. – 45 CFR 126.430

Each vessel over 100 GT must have NAV lights in compliance with applicable nav rules. Anchor light was damaged and not working.

Electrical installations within 16ft of a PV valve must meet requirements for class I division I locations. Lighting on port and starboard side adjacent to the methanol PV valves are not rated for class I div I. Vessel may not carry methanol in tanks until correct lighting is installed.

Each rudder angle indicator must have a transmitter that transmits rudder angle to a remote indicator. Indicator on the bridge for the starbord rudder was 9 degrees off the actual angle.

Each OSV must meet emergency communications requirements in Sub J. PA and GA on deck were inoperable on emergency power.

Each vsl must carry approved life buoys. Port side bridge life buoy found to be not serviceable. Replace life buoy.

On vessels required to have a final emergency power source by 46 CFR 112.05-5(a), the following emergency lighting and power loads mentioned in this cite must be arranged so that they can be energized from the final emergency power source. During inspection, inspectors simulated dark ship condition, disconnecting all ship service generators from main bus on switchboard in ECR. Emergency generator came online within 7 seconds but did not connect to bus on emergency switchboard. Engineers on board could not connect emergency generator to switchboard bus to take a load or supply the required power to the emergency lighting and power loads required in 46 CFR 112.15-5. Vessel shall make satisfactory repairs to the switchboard prior to movement. Vessel not authorized to get underway until deficiency is cleared by attending MI.
