The owner or managing operator of a towing vessel must establish procedures to address fall overboard prevention and recovery of persons in the water, including, but not limited to: 1) Personal Protective equipment, 2) Safely working on the tow 3) Safety while line handling, 4) Safely moving between the vessel and a tow, pier, structure, or other vessel; and 5) Use of retrieval equipment. (b) The owner, managing operator or master must ensure that all persons on board comply with the policies and procedures in this section. Received report of crew member falling overboard during vessel operations without proper safety PPE and sustaining injuries. Vessel to submit copy of SMS procedures attesting to presence of safety procedures on board and report from TPO indicating vessel crew members have received safety training and have acknowledged conformity prior to departure. 46 CFR 140.425

The master must ensure that all life-saving equipment is maintained and ready for use at all times. Vessel had a 12 person life-raft on starboard side in place of the required 25 person life-raft. Inspector found 25 person life-raft located on adjoining barge stowed improperly. Provide evidence that 25 person life-raft is stowed properly on-board to the satisfaction of TPO or CG.
